单孔腹腔镜联合硬膜外穿刺针治疗小儿腹股沟斜疝的临床疗效

摘　　要：目的探讨单孔腹腔镜联合硬膜外穿刺针治疗小儿腹股沟斜疝的临床疗效。方法96例腹股沟斜疝患儿为研究对象,均采用单孔腹腔镜联合硬膜外穿刺针治疗,观察临床效果、手术时间、住院时间及术后并发症发生情况。结果96例患儿均顺利完成手术,单侧腹股斜沟疝平均手术时间(14.0±4.1)min,双侧腹股沟斜疝平均手术时间(21.0±5.9)min,住院时间1~4 d、平均住院时间(2.03±0.85)d。术后1周开始随访,采用门诊复诊与电话随访相结合,随访时间4个月~2年。无切口感染、戳孔疝、线结异物反应、医源性损伤、复发等病例。结论采用单孔腹腔镜联合硬膜外穿刺针治疗小儿腹股沟斜疝的疗效显著,手术时间短,手术创伤小,瘢痕隐蔽,术后并发症少,建议推广。Objective To discuss the clinical efficacy of single-port laparoscopy combined with epidural puncture needle in the treatment of indirect inguinal hernia in children.Methods 96 children with indirect inguinal hernia were selected as the research subjects,and all of them were treated with single-port laparoscopy combined with epidural puncture needle.The clinical effect,operation time,hospital stay,and postoperative complications were observed.Results All 96 children completed the surgery successfully,and the average operation time was(14.0±4.1)min for unilateral inguinal hernia and(21.0±5.9)min for bilateral inguinal hernia,and the hospital stay was 1-4 d,with an average of(2.03±0.85)d.Follow-up began 1 week after surgery,using a combination of outpatient follow-up and telephone follow-up,with a follow-up period of 4 months-2 years.There were no cases of incision infection,puncture hernia,foreign body reaction to the wire knot,medically induced injury,and recurrence.Conclusion Single-port laparoscopy combined with epidural puncture needle has significant efficacy in the treatment of indirect inguinal hernia in children,with short operation time,less trauma,hidden scar and less postoperative complications.It is suggested to be promoted.
