二甲双胍联合炔雌醇环丙孕酮治疗多囊卵巢综合征的效果分析

摘　　要：目的分析多囊卵巢综合征患者采用二甲双胍联合炔雌醇环丙孕酮治疗的临床效果。方法90例多囊卵巢综合征患者,采用电脑系统随机排序分为对照组和观察组,每组45例。对照组患者采用炔雌醇环丙孕酮治疗,观察组患者采用二甲双胍联合炔雌醇环丙孕酮治疗。对比两组患者性激素指标[促黄体生成素(LH)、促卵泡生成素(FSH)、雌二醇(E2)、睾酮(T)]、胰岛功能指标[空腹胰岛素(FINS)和稳态模型胰岛素抵抗指数(HOMA-IR)]水平及治疗效果。结果治疗后,观察组患者LH(9.44±1.75)U/L、T(1.44±0.43)nmol/L低于对照组的(12.71±2.02)U/L、(3.13±0.34)nmol/L,FSH(6.96±1.02)U/L、E2(155.36±14.52)pmol/L高于对照组的(6.25±1.20)U/L、(107.46±7.56)pmol/L,差异具有统计学意义(P<0.05)。治疗后,观察组患者FINS水平为(8.21±1.02)μIU/ml、HOMA-IR为(1.28±0.22),低于对照组的(12.54±2.96)μIU/ml、(2.36±0.45),差异具有统计学意义(P<0.05)。观察组患者治疗总有效率为91.11%(41/45),高于对照组的75.56%(34/45),差异具有统计学意义(P<0.05)。结论采用二甲双胍联合炔雌醇环丙孕酮治疗多囊卵巢综合征效果显著,患者性激素指标得到改善,具有临床推广应用价值。Objective To analyze the clinical effect of metformin combined with ethinylestradiol and cyproterone acetate in the treatment of polycystic ovarian syndrome.Methods A total of 90 patients with polycystic ovary syndrome were randomly sorted into control group and observation group by computerized system,with 45 cases in each group.Patients in the control group were treated with ethinylestradiol and cyproterone acetate,and patients in the observation group were treated with metformin combined with ethinylestradiol and cyproterone acetate.Both groups were compared in terms of levels of sex hormone indexes[luteinizing hormone(LH),follicle stimulating hormone(FSH),estradiol(E;),testosterone(T)],islet function indexes[fasting insulin(FINS)and homeostatic model assessment for insulin resistance(HOMA-IR)],and therapeutic effects.Results After treatment,the LH(9.44±1.75)U/L and T(1.44±0.43)nmol/L in observation group were lower than(12.71±2.02)U/L and(3.13±0.34)nmol/L in control group,and FSH(6.96±1.02)U/L and E;(155.36±14.52)pmol/L were higher than(6.25±1.20)U/L and(107.46±7.56)pmol/L in the control group.All the differences were statistically significant(P<0.05).After treatment,the level of FINS in the observation group was(8.21±1.02)μIU/ml,and the HOMA-IR was(1.28±0.22),which were lower than(12.54±2.96)μIU/ml and(2.36±0.45)in the control group,and the differences were statistically significant(P<0.05).The total effective rate in the observation group was 91.11%(41/45),which was higher than 75.56%(34/45)in the control group,and the difference was statistically significant(P<0.05).Conclusion Metformin combined with ethinylestradiol and cyproterone acetate has a significant effect on polycystic ovarian syndrome,and the sex hormone index is improved,which contains the value of clinical promotion and application.
