超脉冲点阵CO_(2)激光联合强脉冲光在皮肤年轻化治疗中的效果及安全性分析

摘　　要：目的:探讨超脉冲点阵CO_(2)激光联合强脉冲光在皮肤年轻化治疗中的效果及安全性。方法:选取2018年5月-2019年12月笔者医院就诊的110例面部老化行皮肤年轻化治疗的就医者,依照治疗方法将采用单纯超脉冲点阵CO_(2)激光治疗的52例就医者纳入B组,将采用超脉冲点阵CO_(2)激光联合强脉冲光治疗的58例就医者纳入A组,比较两组临床疗效、Visia检测分值、皮肤光泽度与弹性、皮肤屏障功能以及不良反应发生情况。结果:两组临床疗效比较,差异无统计学意义(P>0.05);A组显效率为43.10%,高于B组的23.08%,差异有统计学意义(P<0.05);治疗后3个月、6个月,A组VISIA皮肤图像分析仪指标中紫质、棕色斑、紫外线色斑及斑点计数低于B组,差异有统计学意义(P<0.05);治疗后3个月、6个月,A组皮肤光泽度与弹性均高于B组,差异有统计学意义(P<0.05);两组治疗后3个月、6个月,表皮水分丢失量(TEWL)、角质层含水量及表皮皮脂含量比较,差异无统计学意义(P>0.05);两组就医者治疗后均有不同程度的红肿、疼痛及红斑,均在短时间内消退,A组有2例术后色素沉着,B组有3例术后色素沉着。结论:超脉冲点阵CO_(2)激光联合强脉冲光可有效改善面部皮肤老化症状,对面部色斑、皮肤光泽度及弹性的改善效果优于单纯超脉冲点阵CO_(2)激光治疗,是一种有效的面部年轻化治疗方法,联合使用强脉冲光未增加不良反应风险。Objective To explore the application effect and safety of ultra-pulse fractional CO_(2)laser combined with intense pulsed light in skin rejuvenation.Methods A total of 110 patients with facial aging who underwent skin rejuvenation treatment in the hospital from May 2018 to December 2019 were enrolled.According to different treatment methods,52 cases who underwent ultra-pulse fractional CO_(2)laser were included in group B,while another 58 cases who underwent ultra-pulse fractional CO_(2)laser combined with intense pulsed light were included in group A.The clinical curative effect,scores of Visia test,skin gloss and elasticity,skin barrier function and the occurrence of adverse reactions were compared between the two groups.Results There was no significant difference in clinical curative effect between the two groups(P>0.05).The marked effective rate in group A was higher than that in group B(43.10%vs.23.08%)(P<0.05).At 3 month and 6 months after treatment,counts of poryphyrins,brown spots,ultraviolet spots and spots in VISIA skin image analyzer of group A were lower than those of group B(P<0.05),skin gloss and elasticity were higher than those of group B(P<0.05)At 3 month and 6 months after treatment,there were no significant differences in transepidermal water loss(TEWL),water content of stratum corneum and content of epidermal sebum between the two groups(P>0.05).After treatment,there was redness,pain and erythema of different degree in both groups,which were all resolved in a short time.After surgery,there were 2 cases with pigmentation in group A,and 3 cases with pigmentation in group B.And the fading was not significant.Conclusion Ultra-pulse fractional CO_(2)laser combined with intense pulsed light can effectively improve the symptoms of facial skin aging,whose improvement effects are better than those of pure ultra-pulse fractional CO_(2)laser on facial spots,skin gloss and elasticity.It is a safe and effective treatment method for facial rejuvenation.Combined usage of intense pulsed light will not increas
