皮肤牵张闭合器联合负压创面治疗技术在慢性创面修复中的应用

摘　　要：目的:探讨皮肤牵张闭合器联合负压创面治疗技术治疗慢性创面的可行性、有效性及安全性,为其临床的推广应用提供实验依据。方法:本研究选取住院进行创面修复治疗并符合纳入标准的50例患者,随机分为观察组及对照组,每组25例。观察组给予皮肤牵张闭合器联合负压创面治疗技术治疗,对照组按常规给予负压创面治疗技术治疗,待创面清洁、肉芽新鲜后通过二期手术植皮或局部皮瓣转移修复创面。对两组受试对象创伤及恢复情况,住院费用以及满意度等进行比较。结果:50例患者创面全部治愈,观察组直接闭合创面8例、二期植皮修复13例、局部皮瓣转移修复4例;对照组通过二期植皮修复创面21例、局部皮瓣转移修复4例。与对照组受试对象比较,观察组患者创基准备时间、创面愈合时间更少,差异有统计学意义(P<0.05)。观察组患者治疗费用、住院时间均明显少于对照组(P<0.05)。观察组患者的满意度高于对照组(P<0.05)。结论:皮肤牵张闭合器联合负压创面治疗技术治疗慢性创面患者具有效果可靠、创伤较小、并发症少等优点,值得临床推广应用。Objective To explore the feasibility,effectiveness and safety of skin stretch closure combined with negative pressure wound treatment technology in the treatment of chronic wounds,and to provide experimental evidence for its clinical promotion and application.Methods In this study,50 patients who were hospitalized and met the inclusion criteria were randomly divided into observation group and control group,with 25 cases in each group.The observation group was treated with skin stretch closure combined with negative pressure wound treatment technology,and the control group treated with treatment as usual.After the wound was cleaned and the granulation was fresh,the wound was repaired by second-stage skin grafting or local flap transfer.Then the wound healing time,treatment costs,hospitalization time,and patient satisfaction,etc were compared between groups.Results In the observation group,there were 8 cases of direct wound closure,13 cases of secondary skin grafting and 4 cases of local flap transfer.In the control group,21 cases of wounds were repaired by secondary skin grafting and 4 cases were repaired by local flap transfer.Compared with the subjects in the control group,the observation group had less time for foundation preparation and wound healing,and the difference was statistically significant(P<0.05).The treatment cost and hospital stay in the observation group were significantly less than those in the control group(P<0.05).The satisfaction of patients in the observation group was higher than that in the control group(P<0.05).Conclusion Skin stretch closure combined with negative pressure wound treatment technology has the advantages of reliable effect,less trauma,and fewer complications in the treatment of chronic wounds,and it is worthy of clinical application.
