全程综合保温措施在小儿大面积烧伤清创植皮术围术期的应用效果观察

摘　　要：目的:探讨全程综合保温措施在小儿大面积烧伤清创植皮术围手术期的应用效果。方法:以2017年2月-12月收治的20例患儿为常规组,2018年1月-12月收治的29例患儿为全程综合保温组,对比两组患儿术前、术中体温、术中低体温持续时间和出血量、术后寒战发生率和苏醒时间以及2周后创面愈合率。结果:全程综合保温组术前、术中体温高于常规组,差异具有统计学意义(P<0.05);术中低体温持续时间、苏醒时间、术中出血量均小于常规组(P<0.05);术后寒战发生率低于常规组(χ^(2)=13.878,P<0.05);两周后创面愈合率优于常规组(t=10.04,P<0.001)。结论:全程综合保温措施可减少低体温发生,缩短术中低体温持续时间,减少术中出血量和术后寒战发生率,提高创面愈合率。Objective To explore the effect of comprehensive thermal insulation measures in pediatric patients with a large area of burn.Methods 49 patients with large area burns from February 2017 to December 2018 was selected as the research subjects.The patients were randomly divided into control group and observation group.20 patients in the control group received routine care,29 patients in the observation group received comprehensive thermal insulation measures.The parameters such as the preoperative and intraoperative temperature,the duration of intraoperative hypothermia and blood loss,the incidence of postoperative chills and recovery time,and the wound healing rate after 2 weeks were compared between the two groups.Results The preoperative and intraoperative temperature of the observation group was higher than that of the control group(P<005),the duration of intraoperative hypothermia,recovery time,and intraoperative blood loss were all lower than those in the control group(P<005),the incidence of postoperative chills was lower than that in the control group(χ^(2)=13.878,P<0.05),the wound surface after 2 weeks The healing rate was better than that of the control group(t=10.04,P<0001).Conclusion The comprehensive thermal insulation measures can reduce the occurrence of hypothermia,shorten the duration of intraoperative hypothermia,reduce the amount of intraoperative blood loss and the incidence of postoperative chills,and improve the wound healing rate.
