摘　　要：目的:探讨前瞻性主动护理模式对四肢开放性骨折并皮肤软组织缺损患者创面修复、心理状况的影响。方法:选取2019年5月-2020年9月期间于笔者医院收治的84例四肢开放性骨折并皮肤软组织缺损患者作为研究对象,依据奇偶数法随机将入组患者分配为观察组和对照组,42例/组。对照组采用常规护理,观察组在对照组的基础上采用前瞻性主动护理模式。对比两组患者干预期间的二次植皮修复率、三次植皮修复率、创面愈合时间、住院时间,干预前后的焦虑自评量表(SAS)和抑郁自评量表(SDS)评分,干预期间并发症发生率和干预后的护理满意度。结果:干预后,观察组患者的SAS评分、SDS评分、二次植皮修复率、创面愈合时间和住院时间均低于对照组,差异有统计学意义(P<0.05),观察组患者对临床护理的总满意度评分高于对照组(P<0.05);干预期间,观察组并发症发生率低于对照组的并发症发生率(P<0.05)。结论:采用前瞻性主动护理模式护理四肢开放性骨折并皮肤软组织缺损患者可改善患者心理状况,减少并发症的发生,促进创面修复,缩短住院时间,并提高临床护理满意度。Objective To explore the effects of prospective active nursing mode on wound repair and psychological status of patients with open limbs fractures and soft tissue defects.Methods A total of 84 patients with open limbs fractures and soft tissue defects admitted to the hospital from May 2019 to September 2020 were enrolled as the research objects.According to odd-even method,they were randomly divided into observation group and control group,42 cases in each group.The control group was given routine nursing,while observation group was given prospective active nursing mode on basis of control group.The repair rates of the second and third skin graft during intervention,wound healing time,hospitalization time,scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)before and after intervention,incidence of complications during intervention and nursing satisfaction after intervention were compared between the two groups.Results After intervention,SAS and SDS scores,repair rate of the second skin graft,wound healing time and hospitalization time in observation group were all lower than those in control group(P<0.05),and score of overall satisfaction with clinical nursing was higher than that in control group(P<0.05).During intervention,incidence of complications in observation group was lower than that in control group(P<0.05).Conclusion The application of prospective active nursing mode in patients with open limbs fractures and soft tissue defects can improve their psychological status,reduce the occurrence of complications,promote wound repair,shorten hospitalization time and improve clinical nursing satisfaction.
