摘  要:目的研究血清转化生长因子-β1(TGF-β1)、Apelin、细胞色素P45019A1(CYP19A1)水平与多囊卵巢综合征(PCOS)患者内分泌异常之间的关系。方法将本院2018年1月至2021年1月期间收治的PCOS患者(n=80)以及同龄健康女性(n=70)分别作为PCOS组与对照组,再将PCOS组患者分别分为高雄激素(n=49)/非高雄激素(n=31)亚组、脂质代谢正常(n=37)/异常(n=43)亚组,肥胖(n=31)/非肥胖(n=49)亚组,胰岛素抵抗(IR,n=30)/非胰岛素抵抗(NIR,n=50)亚组,分别比较PCOS组与对照组基础内分泌及代谢情况,两组间以及PCOS组各亚组血清之间TGF-β1、Apelin、CYP19A1水平,分析血清TGF-β1、Apelin、CYP19A1水平与PCOS患者内分泌异常之间的相关性。结果(1)PCOS组基础LH(bLH)、bLH/FSH、基础睾酮(bT)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、三油甘脂(TC)、总胆固醇(TG)、低密度脂蛋白(LDL-C)水平均显著高于对照组(P<0.05),基础FSH水平显著低于对照组(P<0.05)。PCOS组血清TGF-β1、Apelin、CYP19A1水平均显著高于对照组(P<0.05)。(2)PCOS亚组分析:高雄激素亚组的血清CYP19A1水平显著高于非高雄激素亚组(P<0.05);脂质代谢异常亚组的血清TGF-β1水平显著高于脂质代谢正常亚组(P<0.05),而两组间血清Apelin、CYP19A1水平无显著性差异(P>0.05);肥胖亚组的血清TGF-β1、Apelin水平均显著高于非肥胖亚组(P<0.05);IR亚组的血清TGF-β1、Apelin水平均显著高于NIR亚组(P<0.05)。(3)Pearson相关性分析提示,PCOS患者血清CYP19A1与TGF-β1、Apelin均呈弱正相关(P<0.05),TGF-β1与Apelin之间无显著相关性(P>0.05)。结论PCOS患者血清TGF-β1、Apelin、CYP19A1水平显著高于健康者,与PCOS患者内分泌紊乱存在密切联系,提示以上指标可能通过促进内分泌紊乱参与PCOS进程。Objective:To investigate the relationship of serum levels of transforming growth factor-β1(TGF-β1),apelin,cytochrome P450 family 19 subfamily A polypeptide 1(CYP19A1)and endocrine abnormalities in polycystic ovary syndrome(PCOS).Methods:Eighty PCOS patients(PCOS group)and 70 healthy women(control group)with matched age were enrolled in our hospital from January 2018 to January 2021.The patients in PCOS group were further divided into:hyperandrogenic(n=49)/non-hyperandrogenic(n=31)subgroup,normal(n=37)/abnormal(n=43)lipid metabolism subgroups,obesity(n=31)/non-obesity(n=49)subgroups,and insulin resistance(IR,n=30)/non-insulin resistance(NIR,n=50)subgroups.The basic endocrine and metabolism were compared between the PCOS group and control group.The serum levels of TGF-β1,apelin and CYP19A1 were compared between the PCOS group and control group,as well as among the subgroups of PCOS group.The correlation between TGF-β1,Apelin,CYP19A1 levels and endocrine abnormalities in patients with PCOS was analyzed.Results:The levels of basal(bLH),bLH/FSH,basal testosterone(bT),fasting insulin(FINS),insulin resistance index(HOMA-IR),triglyceride(TC),total cholesterol(TG)and low-density lipoprotein cholesterol(LDL-C)in PCOS group were significantly higher than those in control group(P<0.05),and basal FSH level(bFSH)was significantly lower than that in control group(P<0.05).Serum levels of TGF-β1,apelin and CYP19A1 in PCOS group were significantly higher than those in control group(P<0.05).Serum CYP19A1 level of hyperandrogenic subgroup was significantly higher than that of non-hyperandrogenic subgroup(P<0.05).Serum TGF-β1 level of abnormal lipid metabolism subgroup was significantly higher than that of normal lipid metabolism subgroup(P<0.05),while no significant difference was found in apelin and CYP19A1 levels between the two subgroups(P>0.05).Serum levels of TGF-β1 and apelin of obesity subgroup were significantly higher than those of non-obesity subgroup(P<0.05).Serum levels of TGF-β1 and apelin levels of IR subgroup
