富血小板血浆宫腔灌注治疗薄型子宫内膜的研究进展

摘　　要：薄型子宫内膜患者体外受精-胚胎移植的临床妊娠率降低、流产率升高,是取消胚胎移植周期的重要原因之一。薄型子宫内膜限制了腺体及生长因子靶受体生成、影响了信号转导。目前临床上可通过雌激素补充、阿司匹林、血管活性药物等改善子宫内膜厚度与内膜下血流,但上述方法对部分顽固性薄型子宫内膜治疗效果欠佳,而富血小板血浆(PRP)对部分顽固性薄型子宫内膜有一定治疗作用,成为治疗此类患者的热点。其机制可能是PRP在被激活的过程中释放多种生长因子,协同发挥促炎、趋化、促血管生成和抗凋亡的作用,从而促进薄型子宫内膜的修复和增厚,进而改善临床妊娠结局。本文对PRP的制备方法、作用机制以及治疗薄型子宫内膜的研究进展做一综述,以期为PRP宫腔灌注的临床应用提供更多的理论依据。Thin endometrium decreases the clinical pregnancy rate and increases the abortion rate in patients undergoing IVF-ET,and it is also one of the important reasons for canceling embryo transfer cycle.Thin endometrium limits the production of glands and growth factor target receptors,and affects signal transduction.At present,estrogen supplementation,aspirin,vasoactive drugs,etc.can be used to improve the endometrial thickness and sub-endometrial blood flow.However,the above methods are not effective in the treatment of some intractable thin endometrium.Platelet-rich plasma(PRP)has a certain therapeutic effect on some intractable thin endometrium,and has become a hot spot in the treatment of such patients.The mechanism of PRP in the treatment of thin endometrium may be that PRP releases a variety of growth factors during the activation process,which synergistically plays the role of pro-inflammatory,chemotaxis,angiogenesis and anti-apoptosis,promotes the repair and thickening of thin endometrium,and improves the clinical pregnancy outcome.This paper reviews the research progress of PRP preparation methods,mechanism of action,and the treatment of thin endometrium,in order to provide more theoretical basis for the clinical application of PRP intrauterine perfusion.
