浙江省医院与患者对医疗机构公益性认知的差异研究

摘　　要：目的:了解医院与患者对医疗机构公益性的认知特点,分析双方认知差异,为提高患者公益性获得感提供建议。方法:分别采用典型事件分析法和叙事法,收集浙江省内医院公益性履职的典型事件和患者感知公益性的就医经历叙事材料,提取和分析其共性特征。结果:收集3家医院共27个典型事例,以及31段患者就医叙事文本。研究发现医院所认知的公益性核心意涵接近慈善的概念,关注的是弱势群体,体现在慈善救助活动中。患者所认知的公益性是个人利益与公共利益的交集,关注的是人人普惠的公益性要素,体现在医疗服务全过程,尤其关心医疗服务的质量。结论:医院与患者的认知存在差异,但也有契合之处。医院可以通过弘扬公益性文化,围绕健康促进推动慈善活动公益化,持续改进医疗健康服务质量,引导患者形成合理就医预期,健全医疗健康服务供给体系等方式实现双方认知契合。Objective:To understand the cognitive characteristics of hospitals and patients on the public welfare of medical institutions,to analyze their cognitive differences,so as to put forward suggestions for improving patients'perception of public welfare.Methods:The typical event analysis and narrative method was used to collect the typical events of hospitals in Zhejiang province and the narrative materials of patients'medical experience,the common characteristics were extracted and analyzed.Results:27 typical cases from 3 hospitals and 31 medical experiences of patients were collected.The results showed that the core meaning of public welfare recognized by hospitals was close to the concept of charity.They focused on the vulnerable groups,and the public welfare was reflected in the charity relief activities.The public welfare perceived by patients was the intersection of personal interests and public interests.They were concerned about the public welfare elements of universal benefit.The public welfare was reflected in the whole process of medical service,especially the quality of medical service.Conclusion:There are differences in the cognition of public welfare between hospitals and patients,but there are also similarities.Hospitals can promote public welfare culture,carry out health service activities,improve the quality of medical and health services,guide patients to form reasonable medical expectations,and improve the medical and health service supply system to achieve cognitive fit with patients.
