摘  要:目的探讨枸橼酸咖啡因联合机械通气治疗极/超低体重儿呼吸窘迫综合征(RDS)的效果。方法选取2019年9月至2020年12月广西壮族自治区南溪山医院收治的60例极/超低体重儿RDS患儿,按随机抽签法分为观察组和对照组,每组各30例。对照组患儿采用常规机械通气治疗,观察组患儿在对照组基础上联合采用枸橼酸咖啡因进行治疗,比较两组患儿治疗前后呼气末正压(PEEP),气道阻力,血气指标,机械通气、给氧、住院时间,以及相关并发症发生情况。结果观察组患儿治疗后的PEEP、气道阻力、动脉血二氧化碳分压(PaCO_(2))水平低于对照组,机械通气、给氧、住院时间短于对照组,动脉血氧分压(PaO_(2))水平高于对照组,差异均有统计学意义(P<0.05)。观察组的并发症总发生率为20.00%,低于对照组的46.67%,差异有统计学意义(P<0.05)。结论枸橼酸咖啡因联合机械通气治疗极/超低体重儿RDS,能够显著改善患儿呼吸功能和血气PaCO_(2)、PaO_(2)指标,缩短机械通气、给氧及住院时间,减少呼吸机相关性肺炎、支气管肺发育不良、心律失常、胃肠功能障碍并发症发生率,提高临床疗效与安全性,值得推广。Objective To investigate the effect of Caffeine Citrate combined with mechanical ventilation in the treatment of respiratory distress syndrome(RDS)of extremely/very low weight infants.Methods A total of 60 extremely/very low weight infants with RDS admitted to Nanxishan Hospital of Guangxi Zhuang Autonomous Region from September 2019 to December 2020 were selected and divided into the observation group(n=30)and the control group(n=30)by draw lots at random.The children in the control group were treated with conventional mechanical ventilation,and the children in the observation group were treated with Caffeine Citrate on the basis of the control group.Positive end-expiratory pressure(PEEP),airway resistance,blood gas indicators before and after treatment,mechanical ventilation time,oxygen administration time,hospital stay,and incidence of related complications were compared between the two groups.Results After treatment,PEEP,airway resistance and partial pressure of carbon dioxide in artery(PaCO2)levels in the observation group were lower than those in the control group,mechanical ventilation time,oxygen administration time and hospital stay were shorter than those in the control group,and arterial partial pressure of oxygen(PaO_(2))level was higher than that in the control group,the differen ces statistically significant(P<0.05).The total incidence rate of com plications in the observation group was 20.00%,which was lower than 46.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion Caffeine Citrate combined with mechanical ventilation in the treatment of RDS in extremely/very low weight infants can significantly improve respiratory function and blood gas PaCO2 and PaO2 indexes,shorten mechanical ventilation,oxygen and hospital stay,and reduce the incidence of complications such as ventilator-associated pneumonia,bronchopulmonary dysplasia,arrhythmia,and gastrointestinal dysfunction,can improve the c linical efficacy and safety,which is worth promoting.
