摘 要:目的探讨高频振荡机械通气联合注射用牛肺表面活性剂治疗新生儿急性肺损伤/急性呼吸窘迫综合征的价值。方法选择2020年10月至2021年7月嘉祥县人民医院收治的80例急性肺损伤/急性呼吸窘迫综合征患儿为研究对象,按照随机数字表法分为两组,每组各40例。所有患儿均使用注射用牛肺表面活性剂,对照组使用常频机械通气,观察组实施高频振荡机械通气,比较两组的呼吸支持治疗时间、入住新生儿监护室时间和总住院治疗时间,统计两组治疗期间的并发症发生情况及救治成功率。结果观察组的气管插管机械通气时间、面罩给氧时间和鼻导管吸氧时间短于对照组,差异均有统计学意义(P<0.05)。观察组入住新生儿监护室时间和总住院治疗时间短于对照组,差异均有统计学意义(P<0.05)。观察组脓毒血症、循环衰竭、气胸的总发生率低于对照组,救治成功率高于对照组,差异均有统计学意义(P<0.05)。结论高频振荡机械通气联合注射用牛肺表面活性剂治疗新生儿急性肺损伤/急性呼吸窘迫综合征,能有效缩短患儿氧疗时间和住院时间,显著减少并发症,提高临床治疗效果。Objective To explore the value of high-frequency oscillatory ventilation combined with Calf Pulmonary Surfactant for Injection in the treatment of neonatal acute lung injury/acute respiratory distress syndrome.Methods A total of 80 cases with acute lung injury/acute respiratory distress syndrome from October 2020 to July 2021 in Jiaxiang People’s Hospital were selected and divided into two groups according to random number table method,40 cases in each group.The control group used normal frequency mechanical ventilation,the observation group implemented highfrequency oscillation mechanical ventilation,then the respiratory support treatment time,the time of admission to the neonatal intensive care unit and total hospitalization time were compared between the two groups,and the incidence of complications and the success rate of treatment were counted during the treatment between the two groups.Results The mechanical ventilation time of tracheal intubation,oxygen supply time of mask and oxygen inhalation time of nasal catheter in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).The time of admission to the neonatal intensive care unit and total hospitalization time in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).The total proportion of sepsis,circulatory failure and pneumothorax in the observation group was lower than that in the control group,and the success rate of treatment in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion High-frequency oscillatory mechanical ventilation combined with Calf Pulmonary Surfactant for Injection can effectively shorten the time of oxygen therapy and hospitalization,significantly reduce the complications and improve the clinical effect of treatment for neonatal acute lung injury/acute respiratory distress syndrome.
