银屑病患者接受抗肿瘤坏死因子α制剂治疗前后ANA、抗dsDNA抗体和ENA抗体的变化研究

摘　　要：目的分析抗肿瘤坏死因子α制剂对银屑病患者自身抗体产生的影响。方法选取安阳市人民医院皮肤科收治的86例银屑病患者,按照随机数字表法分成观察组(43例)与对照组(43例),对照组给予阿维A胶囊治疗,观察组给予依那西普治疗,比较两组治疗有效率、治疗前后银屑病皮损面积和严重程度指数(PASI)评分、红细胞沉降率(ESR)、C反应蛋白(CRP)、皮肤病生活质量指标调查表(DLQI)评分及关节受累情况,定期检测患者治疗前后抗核抗体(ANA)、抗dsDNA抗体和可提取性核抗原(ENA)抗体的变化。结果观察组与对照组相比治疗有效率明显更高(P<0.05);两组治疗后PASI评分、ESR、CRP水平、DLQI评分有明显降低(P<0.05),观察组与对照组相比明显更低(P<0.05);两组治疗后肿胀关节数、压痛关节数均有明显减少(P<0.05),观察组与对照组相比明显更少(P<0.05);观察组与对照组相比抗体发生率明显更低(P<0.05)。结论抗肿瘤坏死因子α制剂治疗银屑病能够减轻患者临床症状,缓解关节受累情况,疗效显著,部分患者会出现ANA、抗dsDNA抗体和ENA抗体,但发生率较低。Objective To analyze the effect of anti-tumor necrosis factorαpreparation on autoantibodies in patients with psoriasis.Methods A total of 86 patients with psoriasis treated in Anyang People's Hospital were randomLy divided into observation group(43 cases)and control group(43 cases)according to the random number table method.The control group was treated with acitretin capsules,the observation group was treated with Eetanercept.The effective rate,psoriatic lesions area and severity index(PASI)score,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),skin quality of life questionnaire(DLQI)score and joint involvement were compared between the two groups before and after treatment.The changes of antinuclear antibody(ANA),anti-DSDNA antibody and extractable nuclear antigen(ENA)antibody were detected periodically before and after treatment.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05);PASI score,ESR,CRP level and DLQI score were significantly decreased in both groups after treatment(P<0.05),the observation group was significantly lower than the control group(P<0.05);After treatment,the number of swollen joints and tender joints in both groups decreased significantly(P<0.05),the observation group was significantly less than the control group(P<0.05);After treatment,the number of swollen joints and tenderness joints in the two groups were significantly reduced(P<0.05),and the incidence of antibody in the observation group was significantly lower than that in the control group(P<0.05),Conclusion Anti-tumor necrosis factorαpreparation can relieve clinical symptoms and joint involvement of patients with psoriasis,with significant efficacy.ANA,anti-DSDNA antibody and ENA antibody may occur in some patients,but the incidence is low.
