摘　　要：目的:对比单次静脉注射和持续静脉泵注右美托咪定两种方式对腹腔镜子宫肌瘤剔除术患者血流动力学的影响。方法:将60例腹腔镜子宫肌瘤剔除术患者随机分为观察组(n=30)和对照组(n=30)。麻醉诱导前对照组给予持续静脉泵注右美托咪定,观察组给予单次静脉注射右美托咪定。比较两组患者麻醉效果、不同时间段心率(HR)和平均动脉压(MAP)以及不良反应的发生情况。结果:观察组苏醒时间和拔管时间均低于对照组;气腹建立后观察组HR和MAP水平低于对照组;不良反应发生率低于对照组(均P<0.05)。结论:对腹腔镜下子宫肌瘤剔除术患者给予单次静脉注射右美托咪定更利于维持患者的血流动力学稳定,且用药安全性较高。Objective:To compare the effects of single intravenous injection and continuous intravenous pump injection of dexmedetomidine on hemodynamics in patients undergoing laparoscopic myomectomy.Methods:Sixty patients undergoing laparoscopic myomectomy were randomly divided into an observation group(n=30)and a control group(n=30).Before anesthesia induction,the control group received continuous intravenous infusion of dexmedetomidine and the observation group received single intravenous infusion of dexmedetomidine.The anesthesia effect,heart rate(HR)at different time,mean arterial pressure(MAP)and incidence of adverse reactions were compared between the two groups.Results:The time for recovery and extubation was shorter and the levels of HR and MAP after the establishment of pneumoperitoneum and the incidence of adverse reactions were lower in the observation group than the control group(all P<0.05).Conclusion:A single intravenous administration of dexmedetomidine to patients undergoing laparoscopic hysteromyomectomy is beneficial to maintain hemodynamic stability and high safety.
