摘　　要：目的:探讨重组人血小板生成素(rhTPO)联合免疫抑制(IST)治疗对儿童重型再生障碍性贫血(SAA)的疗效及对细胞因子的影响。方法:选取2017年2月~2021年2月医院收治的SAA患儿64例,随机分为两组,32例/组,对照组接受IST治疗,观察组接受rhTPO联合IST治疗,对比两组临床疗效、血液因子[血小板(plt)、血红蛋白(Hb)、网织红细胞绝对值(ARC)]水平、输血时间[脱离plt输注天数、脱离红细胞(RBC)输注天数]、细胞因子[白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)和干扰素-γ(IFN-γ)]水平。结果:两组治疗有效率无明显差异(P>0.05);两组plt、Hb、ARC、IL-4、IL-10水平随着时间延长而升高,且观察组更高(P<0.05);观察组脱离plt、RBC输注时间均较对照组更长(P<0.05);两组IL-2、IFN-γ水平随着时间延长而降低,且观察组更低(P<0.05)。结论:rhTPO联合IST可有效提高SAA儿童疗效果,改善血常规指标及细胞因子水平,具有临床应用价值。Objective:To investigate the effect of recombinant human thrombopoietin(rhTPO)combined with immunosuppression(IST)on children with severe aplastic anemia(SAA)and its influence on cytokine expression.Methods:64 children with SAA admitted to our hospital from February 2017 to February 2021 were randomly divided into two groups,with 32 cases in each group.The control group received IST treatment,while the observation group received rhTPO combined with IST treatment.The clinical efficacy,blood factor levels[platelet(plt),hemoglobin(Hb),absolute value of reticulocyte(ARC)],transfusion time[days of transfusion from plt,days of transfusion from RBC],cytokines[interleukin-2(IL-2)]were compared between the two groups.Results:There was no significant difference in therapeutic effective rate between the two groups(P>0.05).The levels of plt,Hb,ARC,IL-4,and IL-10 in the two groups increased with time,and the observation group was higher(P<0.05).The observation group's time to leave plt and RBC infusion was longer than that of the control group(P<0.05).The levels of IL-2 and IFN-γin the two groups decreased with time,and the observation group was lower(P<0.05).Conclusion:rhTPO combined with IST can effectively improve the therapeutic effect of SAA children,improve blood routine indexes and cytokine levels,and has clinical application value.
