摘 要:目的 探讨肌电图定位定量电刺激在烧伤后周围神经损伤患者康复治疗中的临床效果。方法 选取2017年8月至2020年8月郑州大学第一附属医院收治的68例烧伤后周围神经损伤患者作为研究对象,按照随机数表法将其随机分为观察组和对照组,每组34例,两组患者在常规药物治疗的同时,观察组患者采用肌电图定位定量电刺激治疗,对照组患者采用低频电疗治疗,对比两组患者临床疗效以及受损神经诱发电位波幅、运动潜伏期与运动神经传导速度(MNCV)结果 治疗结束后.观察组患者优20例、良12例、差2例,明显优于对照组患者的优12例、良14例、差8例(Z=-2.279,P=0.023);观察组患者受损神经诱发电位波幅、MNCV均明显高于对照组,运动潜伏期明显短于对照组(t=10.891、8.253、8.123,P均<0.001)。结论 肌电图定位定量电刺激能有效促进烧伤后周围神经损伤患者神经功能恢复,提高神经肌肉兴奋性,疗效显著,值得临床推广应用。Objective To discuss the clinical efficacy of positioned electrical stimulation triggered by quantitative electromyography on the rehabilitation of patients with peripheral nerve injury after burns.Methods 68 patients with postburn peripheral nerve injury,admitted into The First Affiliated Hospital of Zhengzhou University from August 2017 to August2020,were enrolled as research subjects and divided,using the random number table,into observation group(n=34) and control group(n=34).Besides the conventional treatments,patients in observation group were additionally treated with positioned electrical stimulation triggered by quantitative electromyography,and patients in control group with low-frequency electrotherapy.The clinical efficacy,and evoked potential amplitude motor latency and motor nerve conduction velocity(MNCV) of injured nerves were compared between the two groups.Results After treatment,the clinical efficacy was evaluated as excellent in 20 cases,good in 12 cases,and poor in 2 cases in observation group,much better than control group(Z=-2.279,P=0.023)-excellent in 12 cases,good in 14 cases and poor in 8 cases.The evoked potential amplitude and MNCV level of injured nerves in observation group were significantly higher than control group,whereas the motor latency was much shorter(t=10.891,8.253 and 8.123,all P <0.001).Conclusion Positioned electrical stimulation triggered by quantitative electromyography can effectively promote the functional recovery of injured nerves of patients with peripheral nerve injury after burns and elevate neuromuscular excitability,and therapeutic effects is definite,deserving to be promoted in clinical practice.
