福辛普利联合氯沙坦治疗2型糖尿病肾病患者的疗效及血管内皮功能的影响

摘　　要：目的:探讨福辛普利联合氯沙坦治疗2型糖尿病肾病患者的疗效及血管内皮功能的影响。方法:选取2019年1月—2020年7月洛阳市周山医院收治的92例2型糖尿病肾病患者作为研究对象,按随机数表法分为对照组和研究组,每组各46例。对照组采用氯沙坦治疗,研究组采用福辛普利联合氯沙坦治疗。对比分析两组患者临床疗效、血管内皮功能及不良反应发生率。结果:研究组治疗总有效率为95.65%,高于对照组的71.74%,差异有统计学意义(χ^(2)=9.638,P<0.05)。两组患者治疗后血管假性血友病因子(vWF)、内皮素-1(ET-1)、血管内皮生长因子(VEGF)水平低于治疗前,且研究组低于对照组,差异有统计学意义(t=6.123、7.899、3.994,P<0.05)。研究组不良反应发生率为4.35%,与对照组的15.22%相比,差异无统计学意义(χ^(2)=1.971,P>0.05)。结论:福辛普利联合氯沙坦治疗2型糖尿病肾病效果较佳,能够促进患者血管内皮功能恢复,安全性较高。Objective:To investigate the effect of fosinopril combined with losartan in the treatment of type 2 diabetic nephropathy and its effect on vascular endothelial function.Methods:92 patients with type 2 diabetic nephropathy admitted to the hospital from January 2019 to July 2020 were randomly divided into two groups, with 46 cases in each group. The control group was treated with losartan, and the study group was treated with fosinopril combined with losartan. The clinical efficacy, vascular endothelial function and incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the study group was 95.65% , higher than 71.74% of the control group, the difference was statistically significant (χ^(2)=9.638, P<0.05). The levels of von Willebrand factor (vWF), endothelin-1 (ET-1), and vascular endothelial growth factor (VEGF) after treatment in the two groups were lower than before treatment, and the study group was lower than the control group, the difference was statistically significant (t=6.123, 7.899, 3.994, P<0.05). The incidence of adverse reactions in the study group was4.35% , compared with 15.22% in the control group, the difference was not statistically significant (χ^(2)=1.971, P>0.05).Conclusion:Fosinopril combined with losartan is effective in the treatment of type 2 diabetic nephropathy, which can promote the recovery of vascular endothelial function and has high safety.
