房性期前收缩诱发阵发性心房颤动患者采用动态心电图、心脏彩超检查的作用研究

摘　　要：目的探讨采用动态心电图、心脏彩超检查对房性期前收缩诱发阵发性心房颤动患者的应用效果。方法选择该院于2018年9月—2020年3月收治的86例房性期前收缩合并阵发性心房颤动患者,以随机数字法将其分为研究组与对照组,每组43例,对照组给予动态心电图进行诊断,研究组给予动态心电图+心脏彩超进行诊断,对比两组患者检查结果。结果研究组阵发性心房颤动、24 h房性期前收缩次数、左心房内径分别为(543.25±28.77)次、(9842.54±65.48)次以及(38.52±4.22)mm均高于对照组的(468.25±24.12)次、(869.25±18.78)次以及(0.00±0.00)mm,差异有统计学意义(t=13.100、863.798、59.856,P<0.05),研究组心室率为(158.45±8.25)%低于对照组的(148.65±6.44)%,研究组的收缩联律间期为(0.564±0.010)短于对照组的(0.688±0.013),差异有统计学意义(t=6.140、49.577,P<0.05),研究组心脏形态学异常情况、诊断准确率分别为88.37%、97.67%,高于对照组的46.51%、81.40%,差异有统计学意义(χ^(2)=17.158、4.468,P<0.05)。结论针对房性期前收缩合并阵发性心房颤动患者,应用动态心电检查+心脏彩超的诊断准确率相对更高,与常规心电相比,对患者阵发性心房颤动情况的捕捉更为敏感,对临床诊断具有积极意义。Objective To explore the application effect of Holter electrocardiogram and cardiac color Doppler ultrasound examination on patients with paroxysmal atrial fibrillation induced by atrial premature contraction.Methods 86 patients with atrial premature contraction complicated with paroxysmal atrial fibrillation treated in the hospital from September 2018 to March 2020 were randomly divided into study group and control group,with 43 cases in each group.The control group was given a dymic electrocardiogram for diagnosis,and the study group was given a dynamic electrocardiogram+cardiac color Doppler ultrasound for diagnosis.Compared the examination results of the two groups of patients.Results Paroxysmal atrial fibrillation,the number of 24 h atrial premature contractions,and the internal diameter of the left atrium in the study group were(543.25±28.77)times,(9842.54±65.48)times,and(38.52±4.22)mm,respectively,higher than those in the control group(468.25±24.12)times,(869.25±18.78)times and(0.00±0.00)mm,the difference was statistically significant(t=13.100,863.798,59.856,P<0.05).The ventricular rate in the study group was(158.45±8.25)%lower than that in the control group(148.65±6.44)%.The combined rhythm interval of the study group was(0.564±0.010)shorter than that of the control group(0.688±0.013),the difference was statistically significant(t=6.140,49.577,P<0.05).The abnormalities of cardiac morphology and diagnostic accuracy in the study group were 88.37%and 97.67%,respectively,which were higher than 46.51%and 81.40%in the control group,the difference was statistically significant(χ^(2)=17.158,4.468,P<0.05).Conclusion For patients with premature atrial contractions and paroxysmal atrial fibrillation,the diagnostic accuracy of Holter+cardiac color Doppler ultrasound is relatively higher.Compared with conventional electrocardiogram,it is more sensitive to capture patients'paroxysmal atrial fibrillation,which has positive significance for clinical diagnosis.
