摘  要:目的探讨左甲状腺素钠治疗对先天性甲状腺功能减低症(congenial hypothyroidism,CH)骨代谢指标的影响。方法选取经新生儿疾病筛查确诊且经过左旋甲状腺素钠规范治疗的CH患儿40例为观察组及与之相配的正常儿童40例作为对照组,观察治疗前及治疗1年后两组儿童的甲状腺功能(TSH、FT_(3)、FT_(4))及骨代谢指标(IGF-1、25羟维生素D、ALP、Ca^(2+)、P^(3+)、TRACP5b)。结果两组治疗前Ca^(2+)、P^(3+)、ALP、25(OH)D_(3)、IGF-1、TRACP5b、FT_(4)、TSH水平相比,差异均有统计学意义(P<0.05)。观察组治疗1年与对照组相比,Ca^(2+)、P^(3+)、ALP、25(OH)D_(3)、TRACP5b水平差异有统计学意义(P<0.05);而IGF-1、FT_(4)、TSH水平差异无统计学意义(P>0.05)。结论CH患儿存在骨代谢的异常改变,需及早干预,对CH进行骨代谢的检查,及时补充钙及维生素D,可减少佝偻病及骨质疏松的发生。Objective To investigate the effect of levothyroxine sodium therapy on bone metabolism indexes of congenial hypothyroidism (CH).Methods To select the neonatal disease screening and diagnosis after sinistral thyroxine sodium standard treatment of children with CH 40 patients as observation group and the matching of 40 cases of normal children as control group,observed before treatment and after treatment 1 thyroid function in two groups of children (TSH,FT_(4),FT_(4)) and bone metabolic indices (IGF-1,25 hydroxy vitamin D,ALP,P^(3+),TRACP5b).Results Before treatment,there were significant differences in ,P^(3+),ALP,25(OH)D;,IGF-1,TRACP5b,FT_(4)and TSH levels between the two groups (P<0.05).Compared with the control group,the levels of ,P^(3+),ALP,25(OH)D;and TRACP5b in the observation group after 1 year of treatment were statistically significant (P<0.05).There were no significant differences in IGF-1,FT_(4) and TSH levels(P>0.05).Conclusion There are abnormal changes in bone metabolism in children treated with CH.It is necessary to intervene early,check the bone metabolism of CH,and supplement calcium and vitamin D in time,which can reduce the occurrence of rickets and osteoporosis.
