甘姜苓术汤结合电针治疗腰椎间盘突出症的临床观察

摘　　要：【目的】观察甘姜苓术汤结合电针治疗腰椎间盘突出症的临床疗效。【方法】将102例腰椎间盘突出症患者随机分为甘姜苓术汤组、电针组和联用组,每组各34例,甘姜苓术汤组给予甘姜苓术汤口服治疗,电针组给予电针治疗,联用组给予甘姜苓术汤和电针联合治疗。14 d为1个疗程,共治疗1个疗程。治疗14 d后,评价3组患者的临床疗效,观察3组患者治疗前后视觉模拟量表(VAS)评分和中医症状积分的变化情况,以及Oswestry腰背、下肢功能障碍评分的情况,比较3组患者治疗前后腰椎前屈、后伸活动度及前屈、后伸最大肌力值的变化情况。【结果】(1)甘姜苓术汤组总有效率为67.65%(23/34),电针组为70.59%(24/34),联用组为91.18%(31/34)。联用组疗效优于电针组和甘姜苓术汤组,差异均有统计学意义(P<0.05)。(2)治疗后,3组患者的VAS评分均明显改善(P<0.05),且联用组在改善VAS评分方面明显优于甘姜苓术汤组与电针组,差异均有统计学意义(P<0.05)。(3)治疗后,3组患者的腰痛和下肢麻痛中医症状积分均明显改善(P<0.05),且联用组在改善腰痛和下肢麻痛中医症状积分方面明显优于甘姜苓术汤组与电针组,差异均有统计学意义(P<0.05)。(4)治疗后,3组患者的腰背-下肢功能障碍评分均明显改善(P<0.05),且联用组在改善腰背-下肢功能障碍评分方面明显优于甘姜苓术汤组和电针组,差异均有统计学意义(P<0.05)。(5)治疗后,3组患者的腰椎前屈、后伸活动度及前屈、后伸最大肌力值均明显改善(P<0.05),且联用组在改善腰椎前屈、后伸活动度及前屈、后伸最大肌力值方面明显优于甘姜苓术汤组和电针组,差异均有统计学意义(P<0.05)。【结论】甘姜苓术汤结合电针治疗腰椎间盘突出症,能明显改善患者的临床症状,改善患者的疼痛程度,提高患者腰椎屈伸功能及最大肌力值,疗效显著。Objective To observe the clinical efficacy of Ganjiang Lingzhu Decoction combined with electroacupuncture in the treatment of lumbar intervertebral disc herniation(LIDH).Methods A total of 102 patients with LIDH were randomly divided into the Ganjiang Lingzhu Decoction group,the electroacupuncture group and the combination group,with 34 patients in each group.Fourteen-day was considered as one course of treatment and a total of one course of treatment was given.After 14 days of treatment,the clinical efficacy of the three groups was evaluated,and the changes in Visual Analogue Scale(VAS)scores and traditional Chinese medicine(TCM)symptom score,as well as the Oswestry lumbar back and lower limb dysfunction score were observed before and after treatment,and the changes of mobility and maximum muscle strength in anterior flexion and posterior extension in the three groups were compared.Results(1)The total effective rate was 67.65%(23/34)in the Ganjiang Lingzhu Decoction group,70.59%(24/34)in the electroacupuncture group and 91.18%(31/34)in the combination group.The efficacy of the combination group was superior to that of the electroacupuncture group and the Ganjiang Lingzhu Decoction group,the difference being statistically significant(P<0.05).(2)After treatment,the VAS scores of patients in all three groups were improved significantly(P<0.05),and the improvement of the combination group was significantly superior to that of the Ganjiang Lingzhu Decoction and the electroacupuncture group,the difference being statistically significant(P<0.05).(3)After treatment,the TCM symptom score of low back pain and lower limb numbness in all three groups were improved significantly(P<0.05),and the improvement of the combination group was significantly superior to that of the Ganjiang Lingzhu Decoction and electroacupuncture groups,the difference being statistically significant(P<0.05).(4)After treatment,the scores of low back-lower limb dysfunction in all three groups were improved significantly(P<0.05),and the improvement of t
