加味栝楼瞿麦汤联合壮医脐穴针刺治疗子宫腺肌症的临床观察

摘　　要：【目的】观察加味栝楼瞿麦汤联合壮医脐穴针刺治疗子宫腺肌症的临床疗效。【方法】将60例子宫腺肌症患者随机分为观察组和对照组,每组各30例。对照组给予桂枝茯苓胶囊口服,观察组给予加味栝楼瞿麦汤联合壮医脐穴针刺治疗。2组患者均治疗3个月经周期。治疗3个月后,观察2组患者治疗前后视觉模拟量表(VAS)评分与月经失血图(PBAC)评分的变化情况,以及子宫体积和子宫内膜厚度的情况,比较2组患者治疗前后血清雌激素(E2)水平及血清糖链抗原(CA125)水平的变化情况。【结果】(1)治疗后,2组患者的VAS、PBAC评分明显改善(P<0.05),且观察组在改善VAS评分方面明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的子宫体积、子宫内膜厚度均明显改善(P<0.05),且观察组在改善子宫体积、子宫内膜厚度方面明显优于对照组,差异均有统计学意义(P<0.05)。(3)治疗后,2组患者的血清E2、CA125水平水平明显改善(P<0.05),且观察组在改善血清E2水平方面明显优于对照组,差异有统计学意义(P<0.05)。【结论】加味栝楼瞿麦汤联合壮医脐穴针刺治疗子宫腺肌症,能够明显改善患者痛经和月经量过多的临床症状,有效降低血清E2和CA125水平,疗效显著。Objective To observe the clinical efficacy of modified Gualou Qumai Decoction combined with umbilical acupuncture of Zhuang medicine in treating adenomyosis(AM).Methods Sixty patients with AM were randomly divided into observation group and control group,with 30 cases in each group.The control group was given oral use of Guizhi Fuling Capsule,and the observation group was given modified Gualou Qumai Decoction combined with umbilical acupuncture of Zhuang medicine.Patients in both groups were treated for 3 menstrual cycles.After 3 months of treatment,the changes of visual analogue scale(VAS)scores and the pictorial blood loss assessment chart(PBAC)scores,and the uterine volume and endometrial thickness in two groups of AM patients were observed before and after treatment.The changes of serum estrogen(E2)level and serum carbohydrate antigen(CA125)level in two groups of AM patients were compared before and after treatment.Results(1)After treatment,VAS and PBAC scores in two groups were significantly improved(P<0.05),and the improvement in VAS scores in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(2)After treatment,uterine volume and endometrial thickness in two groups were significantly improved(P<0.05),and the improvement of the observation group was significantly superior to that of the control group,the difference being statistically significant(P<0.05).(3)After treatment,serum E2and CA125 levels in two groups were significantly improved(P<0.05),and the improvement in serum E2 level of the observation group was significantly superior to that of the control group,the difference being statistically significant(P<0.05).Conclusion Modified Gualou Qumai Decoction combined with umbilical acupuncture of Zhuang medicine exerts significant effect in treating AM by improving the clinical symptoms of dysmenorrhea and menorrhagia,and effectively reducing the levels of serum E2 and CA125.
