基于产后抑郁症病机特点谈提高针刺疗效的取穴思路

摘　　要：目前针灸临床治疗产后抑郁症的选经取穴与治疗原发性抑郁症基本一致,以督脉、手足厥阴经腧穴为主。但产后抑郁症特发于妊娠开始至产后4周内的特殊时段,由于血不养心,经脉虚衰,脏腑失调,导致心神失养,脑神紊乱,脏神失常。内在胞宫受损、血气亏虚是为病之本,外在神志异常是为病之标,故其针灸治疗取经、选穴、施术也必有相异之处。临证时当针对病机特点,注重标本兼治,在取调神腧穴的基础上,兼取任脉、足太阴脾经穴以养血补气、调理冲任,并针对全身症状,辨明受累脏腑之侧重,对证、对症取穴以整体提高针灸疗效。The acupuncture points selection for the clinical treatment of postpartum depression are basically the same as those used for the treatment of primary depression,with the acupoints of governor vessel and hand-foot jueyin channel mainly. However,postpartum depression occurs during the special period from the beginning of pregnancy to four weeks after delivery,manifested as insufficient nourishment for the heart and spirit,disorder of brain spirit and dysfunction of the five vicera due to the insufficient blood for nourishing heart, deficiency on meridian vessel and disorder of vicera. The internal damage in uterus and blood qi deficiency are the root cause of the disease,while the external mental abnormality is the symptom of the disease. Therefore,there must be some differences in the treatment of meridian selection,acupoint selection and operation between both diseases. In view of the pathogenic characteristics,attention should be paid to the treatment of both symptoms and root causes. On the basis of spirit regualtion points, ren meridian and spleen meridian point of foot-taiyin should be taken to nourish blood and replenish qi,and regulate chong and conception channels. In view of systemic symptoms,the focus of the viscera involved was differentiated, and the points were selected according to the syndromes and symptoms so as to improve the overall efficacy of acupuncture.
