针灸治疗神经性皮炎有效性及安全性的Meta分析

摘　　要：【目的】通过对近10年针灸治疗神经性皮炎的临床研究文献进行Meta分析,探讨针灸治疗神经性皮炎的临床疗效,为中医治疗神经性皮炎提供参考。【方法】通过计算机检索中国知网数据库(CNKI)、万方数据知识服务平台(WanFang Data)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库、美国医学在线(PubMed)、荷兰医学文献数据库(Embase)及国际循证医学图书馆(Cochrane Library)等中英文数据库,检索针灸治疗神经性皮炎的随机对照试验(RCT)文献。采用Cochrane风险偏倚评估工具对纳入的文献进行方法学质量评价。由2名研究者各自独立严格按照纳入和排除标准进行纳入研究的质量评价和资料提取,采用RevMan 5.2统计软件进行Meta分析。【结果】最终共纳入9篇文献,共801例患者。Meta分析结果显示,针灸治疗神经性皮炎的临床总有效率高于西医常规治疗[OR=3.01,95%CI(1.90,4.79),P<0.00001],针灸治疗的瘙痒评分低于西医常规治疗[MD=-1.30,95%CI(-2.11,-0.48),P=0.002],针灸治疗在复发率方面具有优势[OR=0.39,95%CI(0.20,0.75),P=0.005],差异具有统计学意义。漏斗图提示存在发表偏倚。【结论】针灸治疗神经性皮炎与常规西医治疗相比具有较好的临床疗效,能够改善瘙痒,降低复发率,值得在临床上推广应用。但目前纳入研究的质量较低,样本量较少,仍需更多高质量的随机对照试验对其进行证实。Objective Through Meta-analysis of the clinical research literatures on acupuncture-moxibustion treatment for neurodermatitis in the past 10 years,the clinical efficacy of acupuncture-moxibustion treatment for neurodermatitis was explored to provide a reference for the treatment of neurodermatitis in Chinese medicine.Methods The randomized controlled trials(RCTs)of acupuncture-moxibustion for neurodermatitis were searched by computer through the China Knowledge Network database(CNKI),WanFang Data Knowledge Service Platform(WanFang Data),Weipu Chinese Science and Technology Journal Database(VIP),China Biomedical Literature Database,US Medicine Online(PubMed),Excerpta Medica Database(Embase)and the International Library of Evidence-Based Medicine(Cochrane Library)and other Chinese and English databases.The included literatures were evaluated for methodological quality using the Cochrane Risk of Bias Assessment Tool.Quality evaluation of included studies and data extraction were performed by two investigators each independently and strictly according to the inclusion and exclusion criteria,and Meta-analysis was performed using RevMan 5.2 statistical software.Results A total of 9 literatures were included,including 801 patients.Meta-analysis results showed that the total clinical effective rate of acupuncture-moxibustion in the treatment of neurodermatitis was higher than that of conventional treatment in western medicine[OR=3.01,95%CI(1.90,4.79),P<0.00001],and the pruritus score of acupuncture-moxibustion was lower than that of conventional treatment in western medicine[MD=-1.30,95%CI(-2.11,-0.48),P=0.002],acupuncture-moxibustion treatment had an advantage in recurrence rate[OR=0.39,95%CI(0.20,0.75),P=0.005],the difference being statistically significant.Funnel plot indicates publication bias.Conclusion Acupuncture-moxibustion has superior clinical efficacy in the treatment of neurodermatitis compared with conventional western medical treatment,in improving pruritus and reducing recurrence rates,which is worth promo
