摘 要:目的:探讨同型半胱氨酸(HCY)与胱抑素C(CysC)血清水平变化的特点对于苯酚烧伤患者早期临床的应用价值。方法:选取2017年1月—2020年12月南开大学附属医院烧伤科收治的符合实验标准的30例苯酚烧伤患者作为研究对象,根据接触苯酚的时间分为:接触时间<1h组、接触时间1~3h组、接触时间>3h组。检测并分析各组血清HCY、CysC值。采用ROC曲线评价二者单项检测以及联合检测对苯酚烧伤早期并发急性肾损伤的诊断效能。结果:随着接触苯酚时间的延长,HCY与CysC值都呈现上升趋势,在接触时间>3h组中二者均呈显著性升高(P<0.05)。logistic回归模型-ROC曲线分析显示二者联合检测时AUC为0.929,敏感性为91.67%,均大于其单项指标诊断效能。结论:患者在烧伤早期血清HCY、CysC值与其接触苯酚的时间密切相关。HCY与CysC联合检测对并发急性肾损伤的早期诊断价值优于其单项检测。Objective:To investigate the characteristics of serum homocysteine(HCY)and cystatin C(CysC)levels in patients with phenol burn.Methods:30 cases of phenol burn patients admitted in the hospital from Januaty 2017 to December 2020 were selected and divided into three groups according to the contact time:contact time<1 h group,contact time 1-3 h group and contact time>3 h group.Serum HCY and CysC values were detected and analyzed.ROC curve was used to evaluate the diagnostic efficacy of single detection and combined detection of HCY and CysC in the early stage of phenol burn complicated with acute kidney injury.Results:HCY and CysC values showed an upward trend with the prolongation of phenol exposure time,and they significantly increased in the contact time>3 h group(P<0.05).Logistic regression model ROC curve analysis showed that AUC was 0.929 and sensitivity was 91.67%when combined detection,which were greater than the diagnostic efficiency of single index.Conclusion:The serum HCY and CysC values of patients in the early stage of burn are closely related to the time of contact with phenol.The combined detection of HCY and CysC is better than single detection in the early diagnosis of acute kidney injury.
