摘　　要：目的探讨来曲唑结合低分子肝素钙联合治疗多囊卵巢综合征(PCOS)不孕症的疗效及其对爱帕琳肽(Apelin)、内脂素-1(Nesfatin-1)水平的影响。方法研究对象选择2018年1月至2019年12月于我院就诊110例PCOS不孕症患者,随机将110例患者分为观察组和对照组,各55例,对照组采用来曲唑促排卵治疗,观察组采用来曲唑联合低分子肝素钙促排卵治疗,比较两组患者临床疗效以及对Apelin、Nesfatin-1水平的影响和不良反应率。结果观察组激素水平、排卵率、妊娠率显著优于对照组(P<0.05)。观察组中各时期子宫内膜厚度、血流指数(FI)增加,子宫内膜螺旋动脉血流阻力指数(RI)及搏动指数(PI)降低,且显著优于对照组(P<0.05)。治疗后血清Apelin、Nesfatin-1水平均下降,且观察组显著低于对照组(P<0.05)。比较两组的不良反应,发生率差异无统计学意义(P>0.05)。结论来曲唑结合低分子肝素钙治疗PCOS不孕症,可增强子宫内膜容受性,提高妊娠率,并可以改善apelin和Nesfatin-1水平,值得在临床推广应用。Objective To investigate the efficacy of letrozole combined with low molecular weight heparin calcium in the treatment of polycystic ovary syndrome(PCOS) infertility and its effect on the levels of Apelin and Nesfatin-1. Methods 110 patients with PCOS infertility who were treated in our hospital from January 2018 to December 2019 wereselected. 110 patients were randomly divided into observation group and control group, each with 55 cases. The control group was treated with letrozole for ovulation induction therapy. In the observation group, letrozole combined with low molecular weight heparin calcium was used to stimulate ovulation. The clinical efficacy and the effects on Apelin and Nesfatin-1 levels and adverse reaction rates were compared between the two groups. Results The hormone level, ovulation rate and pregnancy rate of the observation group were better than those in the control group significantly(P<0.05). In the observation group, the endometrial thickness and blood flow index(FI) increased in each period, and the endometrial spiral artery blood flow resistance index(RI)and pulsatility index(PI) decreased, which were better than those of the control group significantly(P<0.05). After treatment,serum Apelin and Nesfatin-1 levels decreased, and the observation group was significantly lower than the control group(P<0.05). Comparing the adverse reactions between the two groups, there was no statistically significant difference in the incidence(P>0.05). Conclusion Letrozole combined with low molecular weight heparin calcium in the treatment of PCOS infertility can enhance endometrial receptivity, increase pregnancy rate, and improve apelin and Nesfatin-1 levels. It is worthy of clinical application.
