摘　　要：目的探讨血管内栓塞介入治疗前交通动脉瘤的疗效及预后。方法选择2018年1月至2020年2月本院收治的前交通动脉瘤患者87例,分为显微手术组44例与介入组43例。介入组行血管内栓塞术,显微手术组选用血管外显微手术,统计两组患者手术时间、术后自主呼吸恢复时间、出血量及导管拔除时间;观察两组患者术前及术后1个月美国国立卫生研究院卒中量表(NIHSS)、简易智力状态检查量表(MMSE)评分变化情况,并比较两组术后1年的格拉斯哥预后量表(GOS)分级。结果介入组手术时间及导管拔除时间均短于显微手术组,术中出血量多于显微手术组,差异有统计学意义(P<0.05);术后介入组NIHSS低于显微手术组,MMSE评分及GOS分级高于显微手术组,差异有统计学意义(P<0.05)。结论前交通动脉瘤使用血管内栓塞术可有效减短患者治疗时间,改善患者预后,减轻神经功能受损程度。Objective To explore the effect and prognosis of endovascular embolization in the treatment of anterior communicating aneurysm.Methods A total of 87 patients with anterior communicating aneurysm admitted in our hospital from January 2018 to February 2020 were selected and divided into microsurgery group(n=44,extravascular microsurgery)and intervention group(n=43,endovascular embolization).The operation time,postoperative recovery time of spontaneous breathing,blood loss and extubation time in both groups were analyzed.The changes in scores of National Institutes of Health Stroke Scale(NIHSS)and Mini-Mental State Examination(MMSE)before surgery and at 1 month after surgery in both groups were observed.The grades of Glasgow Outcome Scale(GOS)at 1 year after surgery were compared between the two groups.Results The operation time and extubation time in intervention group were shorter than those in microsurgery group,and intraoperative blood loss was more than that in microsurgery group(P<0.05).After surgery,NIHSS score in intervention group was lower than that in microsurgery group,while MMSE score and GOS grading were higher than those in microsurgery group(P<0.05).Conclusion The application of endovascular embolization in anterior communicating aneurysm can effectively shorten treatment time,improve prognosis and reduce neurological impairment in patients.
