摘 要:目的研究特发性肺纤维化患者血清中血管内皮生长因子(VEGF)、转化生长因子-β(TGF-β)及白介素-8(IL-8)、白介素-13(IL-13)水平与肺功能相关性。方法选取北京市隆福医院及海口市人民医院2019年4月至2021年4月特发性肺纤维化患者113例作为研究组,同期健康体检的研究志愿者113例作为对照组。比较两组基线资料,血清VEGF、TGF-β及IL-8、IL-13水平,肺功能指标用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、最大呼气流量(PEF)。分析特发性肺纤维化患者血清VEGF、TGF-β及IL-8、IL-13水平与肺功能相关性。结果两组年龄、性别比、体质指数、伴发基础疾病情况比较,差异均无统计学意义(P>0.05)。研究组血清VEGF、TGF-β及IL-8、IL-13水平均显著高于对照者,差异均有统计学意义(P<0.05)。研究组肺功能指标FVC、FEV1、PEF均显著低于对照者,差异均有统计学意义(P<0.05)。研究组血清VEGF、TGF-β及IL-8、IL-13水平与肺功能指标FVC、FEV1、PEF呈显著负相关(P<0.05)。血清VEGF、TGF-β及IL-8、IL-13联合检测曲线面积为0.861(95%CI:0.663~0.921),检测价值更高(P<0.05)。结论特发性肺纤维化患者血清VEGF、TGF-β及IL-8、IL-13水平与肺功能有显著负相关性,通过监测以上各项指标可以评估患者病情,为该疾病诊治工作提供可靠的参考信息。Objective To analyze the correlation between serum VEGF,TGF-β,IL-8,IL-13levels,and pulmonary function in patients with idiopathic pulmonary fibrosis.Methods 113 patients with idiopathic pulmonary fibrosis in our hospital from April 2019 to April 2021 were selected as the research objects,and 113 cases of research volunteers in the same period of physical examination were selected as the control subjects.The baseline data,serum VEGF,TGF-β,IL-8,IL-13 levels,forced vital capacity(FVC),forced expiratory volume(FEV1)and maximal expiratory flow(PEF)of patients with idiopathic pulmonary fibrosis and healthy controls were compared.The correlation between serum VEGF,TGF-β,IL-8,IL-13levels,and pulmonary function in patients with idiopathic pulmonary fibrosis Objective were analyzed.Results There was no significant difference in age,sex ratio,body mass index,and underlying diseases between the two groups(P>0.05).The serum VEGF,TGF-β,IL-8 and IL-13 levels in patients with idiopathic pulmonary fibrosis were significantly higher than those in the healthy control group,and the differences between two groups were statistically significant(P<0.05).The pulmonary function indexes FVC,FEV1 and PEF in patients with idiopathic pulmonary fibrosis were significantly lower than those in the healthy control group(P<0.05).The levels of serum VEGF,TGF-β,IL-8 and IL-13 in patients with idiopathic pulmonary fibrosis were negatively correlated with the pulmonary function indexes FVC,FEV1 and PEF(P<0.05).The area under curve and confidence interval of serum VEGF,TGF-β,IL-8 and IL-13 were combined detection of0.861(95%CI:0.663~0.921)had higher detection value.Conclusion The serum levels of VEGF,TGF-β,IL-8and IL-13 in patients with idiopathic pulmonary fibrosis were significantly negatively correlated with lung function.Monitoring the above indicators can evaluate the patient’s condition and provide reliable reference information for the diagnosis and treatment of the disease.
