摘  要:目的探讨老年髋部骨折术后纤维蛋白原(FIB)、碱性磷酸酶(ALP)、微循环指标表达意义。方法选取河北省保定市第二中心医院118例老年髋部骨折手术患者,根据术后是否延迟愈合分组,延迟组20例,正常组98例,对比两组FIB、ALP、微循环指标水平,分析FIB、ALP与微循环指标水平的相关性及FIB、ALP、微循环指标与老年髋部骨折术后延迟愈合的关系,绘制ROC曲线,评价FIB和ALP联合微循环指标在老年髋部骨折术后延迟愈合中的检测价值。结果延迟组FIB、血浆黏度、红细胞聚集指数、红细胞刚性指数均高于正常组,ALP水平低于正常组,差异有统计学意义(P<0.05);老年髋部骨折术后延迟愈合患者FIB与微循环指标血浆黏度、红细胞聚集指数、红细胞刚性指数呈正相关关系,ALP与血浆黏度、红细胞聚集指数、红细胞刚性指数呈负相关关系(P<0.05);Logistic回归分析,结果显示FIB、ALP、血浆黏度、红细胞聚集指数、红细胞刚性指数是老年髋部骨折患者术后延迟愈合的影响因素(P<0.05),绘制受试者工作(ROC)曲线,评价FIB、ALP、微循环指标水平对老年髋部骨折术后延迟愈合的预测价值,发现各指标联合预测曲线下面积最大,且>0.9,具有良好预测效能(P<0.05)。结论血液流变学及骨代谢异常是导致老年髋部骨折术后延迟愈合的内在原因,临床可通过检测FIB、ALP及微循环指标,预测术后延迟愈合,积极采取合理治疗措施,有助于改善预后。Objective To investigate the expression and significance of fibrinogen(FIB),alkaline phosphatase(ALP)and microcirculation in elderly patients with hip fracture.Methods Second Central Hospital of Baoding city,Hebei Province 118 elderly patients undergoing hip fracture surgery were divided into groups according to whether postoperative healing was delayed,20 cases in the delayed group and 98 cases in the normal group.The FIB,ALP,and microcirculation index levels of the two groups were compared,and the FIB,ALP and microcirculation index levels were analyzed.Correlation and the relationship between FIB,ALP,microcirculation indexes and delayed union of elderly hip fractures after surgery,ROC curve was drawn,and the value of FIB and ALP combined microcirculation indexes in the detection of delayed union of elderly hip fractures after surgery was evaluated.Results The FIB,plasma viscosity,erythrocyte aggregation index,and erythrocyte rigidity index of the delayed group were higher than those of the normal group,and the ALP level was lower than that of the normal group(P<0.05);In elderly patients with delayed healing after hip fracture surgery,FIB was positively correlated with microcirculation indexes,plasma viscosity,erythrocyte aggregation index,and erythrocyte stiffness index,and ALP was negatively correlated with plasma viscosity,erythrocyte aggregation index,and erythrocyte stiffness index(P<0.05);Logistic regression analysis showed that FIB,ALP,plasma viscosity,erythrocyte aggregation index,and erythrocyte stiffness index were the influencing factors of postoperative delayed healing in elderly patients with hip fracture(P<0.05).The receiver operating(ROC)curve was drawn and evaluated The predictive value of FIB,ALP,and microcirculation index levels for the delayed healing of elderly hip fractures after surgery.It is found that the combined prediction area under the curve of each index is the largest,and it is greater than 0.9,which has good predictive power.Conclusion Conclusion Abnormal blood rheology and bone me
