雷火灸对腹腔镜下胆囊切除术后胃肠功能恢复的临床研究

摘　　要：目的:通过对腹腔镜下胆囊切除术(LC)患者使用“雷火灸”观察其胃肠功能恢复情况,评价临床疗效。方法:将入组患者以数字随机法分为实验组(雷火灸组)和对照组(空白组),每组50例(实验组剔除1例,为49例)。实验组术后2 h立即使用雷火灸灸患者双侧足三里、下巨虚,1次/d,直至排气。对照组术后不予干预,手术后6 h测肠鸣音,1次/2 h,至肠鸣音恢复,同时记录其首次排气排便时间。两组手术第0天(am:6∶00)测胃泌素1次,手术后第1天(am:6∶00)测胃泌素1次,测胃泌素水平。结果与结论:雷火灸组肠鸣音、首次排气、排便时间明显提前,与对照组相比有显著差异(P_(肠鸣音)<0.01,P_(排气)<0.01,P_(排便)<0.01),证明雷火灸组有收益。雷火灸治疗可使术后(第一天)胃泌素处于一个较高水平(↑17.58±11.48pg/ml),显示其明显促进了胃肠道平滑肌的蠕动,让胃肠道恢复时间提前,进而加速胃肠功能的恢复。Objective:To observe the recovery of gastrointestinal function and evaluate the clinical effect of LC patients after operation.Methods:The patients in the group were divided into experimental group(moxibustion group)and control group(blank group),There were 50 cases in each group(49 cases in the experimental group).Experimental group:2 hours after operation,the patients were treated with moxibustion with thunder-fire moxibustion on both sides of"Zusanli","lower giant deficiency",Once a day,Up to exhaust.Control group:no intervention after operation.The bowel sounds were monitored 6 hours after operation,Once in two hours,Until the bowel sounds recover,At the same time record its first exhaust defecation time.Gastrin was measured once on day 0(am:6:00)of the operation in both groups,Gastrin was measured once on day 1(am:6:00)after surgery,The level of gastrin was measured.Results:There were significant differences in bowel sounds,first exhaust and defecation time(P bowel sounds<0.01,P exhaust<0.01,P defecation 0.01).It is proved that the lightning moxibustion group has income.Meanwhile,the treatment of"thunder fire moxibustion"can make the gastrin at a high level(↑17.58±11.48 pg/mL)after operation,which shows that it can obviously promote the peristalsis of gastrointestinal smooth muscle and advance the recovery time of gastrointestinal tract.And then accelerate the recovery of gastrointestinal function.
