疏肝补肾法防治乳腺癌内分泌治疗后类绝经期综合征的Meta分析

摘　　要：目的:系统评价及Meta分析评估疏肝补肾法防治乳腺癌内分泌治疗后类绝经期综合征的疗效。方法:检索中国生物医学文献数据库(CBM)、中国知识资源总库(CNKI)、维普中文科技期刊全文数据库、万方数据学术论文总库、Pubmed数据库,收集2010年1月1日—2019年12月31日发表的有关疏肝补肾法防治乳腺癌内分泌治疗后类绝经期综合征的文献,提取数据。运用RevMan5.3对有效率、中医症候积分、骨密度(BMD)、总胆固醇(TC)、甘油三酯(TG)进行Meta分析,并统计常用中药使用频数。结果:总计纳入21篇文献,病例1617例。Meta分析结果显示,有效率[OR=7.02,95%CI=(4.32,11.40),P<0.00001],差异有统计学意义。中医症候积分[SMD=-0.80,95%CI=(-0.97,-0.63),P<0.00001],差异有统计学意义。骨密度[SMD=0.78,95%CI=(0.64,0.92),P<0.00001],差异有统计学意义。总胆固醇[MD=-0.35,95%CI=(-0.85,0.15),P=0.17]、甘油三酯[MD=-0.22,95%CI=(-0.45,0.00),P=0.06],差异无统计学意义。结论:对比单纯内分泌治疗,联合疏肝补肾法能有效防治内分泌治疗所致类绝经期综合征,改善骨质疏松,但对总胆固醇及甘油三酯无影响。防治潮热盗汗以疏肝补肾、健脾养血为主,治疗骨质疏松以补益肝肾为主。Objective:To systematically evaluate the effect of Shugan-Bushen compounds in dealing with postmenopausal syndrome caused by endocrine therapy in breast cancer.Methods:Search for papers of RCTs from 2010 to 2019 in CBM,CNKI,VIP,WANFANG and PubMed.Collect information about papers related to Shugan-Bushen compounds used in postmenopausal syndrome caused by en-docrine therapy.Then,use RevMan5.3 to process the data and count the frequency of Chinese herbals.Results:21 studies were included,involving 1060 patients.Meta-analysis shows the effective rate[OR=10.17,95%CI=(4.13,25.06),P<0.00001],TCM syndrome score[SMD=-0.80,95%CI=(-0.97,-0.63),P<0.00001],BMD[SMD=0.78,95%CI=(0.64,0.92),P<0.00001],with statistical significance.TC[MD=-0.35,95%CI=(-0.85,0.15),P=0.17]、TG[MD=-0.22,95%CI=(-0.45,0.00),P=0.06],with no statistical significance.Conclusion:Compared to applying endocrine treatments alone,the introduction of Shugan-Bushen compounds makes sense in dealing with post-menopausal syndrome caused by endocrine therapy,which can reduce TCM syndrome scores and increase BMD.However,it has no influence in TC and TG.Species of Chinese herbals tend to be different in different symptoms.
