摘　　要：目的:探究关节镜辅助下微创经皮钢板内固定(MIPPO)术在低能量胫骨平台骨折伴韧带损伤治疗中的应用效果。方法:选择2020年3月至2021年6月广州市番禺区中医院收治的低能量胫骨平台骨折伴韧带损伤患者100例为研究对象。采用随机数表法将其分为对照组与观察组(50例/组)。为对照组患者采用切开复位内固定术进行治疗,为观察组患者采用关节镜辅助下MIPPO术进行治疗。然后比较两组患者的手术指标、骨折复位及愈合的时间、膝关节功能及术后并发症的发生情况。结果:1)观察组患者手术切口的长度短于对照组患者,其术中的出血量少于对照组患者,差异有统计学意义(P<0.05)。两组患者手术的时间相比,差异无统计学意义(P>0.05)。2)治疗后,观察组患者的Rasmussen评分、骨折复位的优良率均高于对照组患者,其骨折愈合的时间短于对照组患者,差异有统计学意义(P<0.05)。3)治疗后,观察组患者的HSS评分、膝关节功能的优良率均高于对照组患者,差异有统计学意义(P<0.05)。4)观察组患者术后并发症的发生率低于对照组患者,差异有统计学意义(P<0.05)。结论:用关节镜辅助下MIPPO术治疗低能量胫骨平台骨折伴韧带损伤的效果显著,具有创伤小、骨折复位效果好、术后并发症少、可显著改善患者的膝关节功能等优点。Objective:To explore the effect of minimally invasive percutaneous plate internal fixation(MIPPO)assisted by arthroscopy in the treatment of low energy tibial plateau fracture with ligament injury.Methods:From March 2020 to June 2021,100 patients with low energy tibial plateau fracture and ligament injury admitted to Panyu Hospital of Traditional Chinese Medicine in Guangzhou were selected as the study subjects.They were divided into control group and observation group(50 cases/group)by random number table method.Patients in the control group were treated with open reduction and inte rnal fixation,and patients in the observation group were treated with arthros copically assisted MIPPO.Then,the surgical indicators,the time of fracture reduction and healing,the knee function and the occurrence of postoperative complications were compared between the two groups.Results:1)The length of surgical incision in the observation group was shorter than that in the control group,and the amount of intraoperative blood loss was less than that in the control group,with statistical significance(P<0.05).There was no significant difference in the operation time between the two groups(P>0.05).2)After treatment,Rasmussen score and excellent and good rate of fracture reduction in observation group were higher than those in control group,and the time of fracture healing was shorter than that in control group,with statistical significance(P<0.05).3)After treatment,HSS score and excellent and good rate of knee function in observation group were higher than those in control group,the differences were statistically significant(P<0.05).4)The incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Arthroscope-assisted MIPPO has a significant effect on the treatment of low energy tibial plateau fracture with ligament injury,which has the advantages of small trauma,good fracture reduction effect,fewer postoperative complications a
