摘 要:目的探讨不同脊柱侧凸类型与胸锁关节病变的相关性。方法回顾性分析自2018-01—2020-01诊治的85例脊柱侧凸,比较各脊柱侧凸类型T1倾斜角、双肩高度差、锁骨角,采用Pearson相关性检验对T1倾斜角、双肩高度差、锁骨角与胸锁关节病变的相关性进行分析。结果85例均获取完整的影像学资料。LenkeⅠ型17例,其中2例合并胸锁关节病变;LenkeⅡ型21例,其中17例合并胸锁关节病变;LenkeⅢ型10例,其中3例合并胸锁关节病变;LenkeⅣ型9例,其中4例合并胸锁关节病变;LenkeⅤ型16例,其中2例合并胸锁关节病变;LenkeⅥ型12例,其中5例合并胸锁关节病变。各脊柱侧凸类型间T1倾斜角差异有统计学意义(P<0.05),而各脊柱侧凸类型间双肩高度差、锁骨角比较差异无统计学意义(P>0.05)。Pesrson相关性分析结果显示,T1倾斜角与胸锁关节病变呈正相关(P<0.05)。双肩高度差、锁骨角与胸锁关节病变无明显相关性(P>0.05)。结论脊柱侧凸同时存在不同程度胸廓变形,胸廓小关节的应力不平衡,其中胸锁关节的顺应性较低,易导致其发生病变。Objective MethodsEighty-five patients with scoliosis diagnosed and treated from January 2018 to January 2020 were analyzed retrospectively.T1tilt angle,shoulder height difference and clavicular angle of each scoliosis type were compared.The correlation between T1tilt angle,shoulder height difference,clavicular angle and sternoclavicular arthritis was analyzed by Pearson correlation test.ResultsComplete imaging data were obtained in 85 cases.There were 17 cases of Lenke typeⅠ,of which 2 cases were complicated with sternoclavicular joint lesions;21 cases of LenkeⅡ,of which 17 cases with sternoclavicular joint lesions;10 cases of LenkeⅢ,including 3 cases with sternoclavicular joint lesions;9 cases of Lenke typeⅣ,including 4 cases with sternoclavicular joint lesions;16 cases of LenkeⅤtype,of which 2 cases with sternoclavicular joint lesions;12 cases of LenkeⅥtype,of which 5 cases with sternoclavicular joint lesions.There was significant difference in T1tilt angle among scoliosis types(P<0.05),but there was no significant difference in shoulder height difference and clavicular angle among scoliosis types(P>0.05).Pesrson correlation analysis showed that T1tilt angle was positively correlated with sternoclavicular joint lesions(P<0.05).There was no significant correlation between shoulder height difference and clavicular angle and sternoclavicular joint disease(P>0.05).ConclusionThere are different degrees of thoracic deformation in scoliosis,and the stress of thoracic facet joint is not balanced.The compliance of sternoclavicular joint is low,which is easy to lead to disease.
