人工肱骨头置换术与锁定钢板内固定治疗四部分肱骨近端骨折疗效的Meta分析

摘　　要：目的通过Meta分析比较人工肱骨头置换术与锁定钢板内固定治疗四部分肱骨近端骨折的临床疗效。方法检索自2000-01—2020-03收录在中国知网、Pubmed等数据库关于比较人工肱骨头置换术(肱骨头置换组)与锁定钢板内固定(锁定钢板组)治疗四部分肱骨近端骨折的文献,采用RevMan 5.3软件进行Meta分析。比较肱骨头置换组与锁定钢板组手术时间、术中出血量、并发症发生率以及末次随访时的Constant-Murley评分、疼痛VAS评分、疗效优良率。结果肱骨头置换组与锁定钢板组在手术时间、并发症发生率以及末次随访时的疼痛VAS评分、疗效优良率方面比较差异无统计学意义(P>0.05)。锁定钢板组术中出血量较肱骨头置换组少,末次随访时Constant-Murley评分较肱骨头置换组高,差异有统计学意义(P<0.05)。结论锁定钢板内固定治疗四部分肱骨近端骨折术中出血量较少,术后可以获得更好的肩关节功能。ObjectiveTo compare the clinical efficacy of hemiarthroplasty and locking plate fixation in the treatment of fourpart proximal humeral fractures by Meta-analysis.MethodsLiterature in Pubmed and other databases on the comparison of hemiarthroplasty(humeral head replacement group) and locking plate internal fixation(locking plate group) in the treatment of four-part proximal humeral fractures was retrieved from January 2000 to March 2020 in CNKI and Meta-analysis was performed using RevMan 5.3 software. The operation time, intraoperative blood loss, incidence of complications, Constant-Murley score, pain VAS score and excellent and good rate of efficacy were compared between the hemiarthroplasty group and the locking plate group.ResultsThere was no significant difference in operation time, incidence of complications, and pain VAS score,excellent and good rate of curative effect at last follow-up between hemiarthroplasty group and locking plate group(P>0.05).The intraoperative blood loss in the locking plate group was less than that in the hemiarthroplasty group, and the Constant-Murley score at the last follow-up was higher than that in the hemiarthroplasty group, the difference was statistically significant(P<0.05).Conclusion bleeding and better postoperative shoulder joint function.
