摘 要:目的探讨血清γ-谷氨酰转肽酶(GGT)和丙氨酸氨基转移酶(ALT)与新诊断高尿酸血症(HUA)患病之间的关联,为HUA的防治提供科学依据。方法于2006年1月和2009年1月采用4阶段分层随机抽样的方法选取青岛地区9148名35~74岁居民为研究对象,进行问卷调查、体格检查和实验室检测。采用SPSS 23.0统计软件进行t检验、MannWhitney U检验和χ^(2)检验,尿酸与GGT、ALT水平的相关性采用Spearman相关分析,新诊断HUA与GGT、ALT水平的关系采用多因素logistic回归分析。结果男性新诊断HUA患病率(19.8%)高于女性(11.7%),差异有统计学意义(P<0.05)。男性GGT、ALT水平高于女性,差异均有统计学意义(P<0.05)。Spearman相关分析结果显示,ALT、GGT水平均与尿酸水平均呈正相关(r值分别为0.175、0.434,P<0.01)。多因素logistic回归分析结果显示,在调整了年龄、体质指数、城乡分布、受教育水平、个人月收入、吸烟及饮酒状况后,男性(OR=1.64,95%CI:1.07~2.51)与女性(OR=2.01,95%CI:1.34~3.00)的ALT异常和男性(OR=2.87,95%CI:2.26~3.65)与女性(OR=2.02,95%CI:1.45~2.81)的GGT异常均与新诊断HUA患病高风险相关。结论青岛市不同性别人群血清肝酶水平异常与新诊断HUA患病高风险相关。Objective To explore the relationship between serumγ-glutamyl transferase(GGT),alanine aminotransferase(ALT)and newly diagnosed hyperuricemia(HUA),and provide the scientific basis for preventing HUA.Methods In January 2006 and January 2009,the multi-stage stratified random sampling method was used to select 9148 residents(35-74 years old)in Qingdao as the subjects.The investigation was performed with questionnaire,physical examination and laboratory test.The used software was SPSS 23.0.The t test,Mann-Whitney U test andχ^(2)test were used to analyze the data.Spearman correlation analysis was used to analyze the correlation between ALT,GGT levels and uric acid.Multivariate logistic regression analysis was used to analyze the relationship between newly diagnosed HUA and GGT and ALT levels.Results The morbidity(19.8%)of newly diagnosed HUA in males was significantly higher than that(11.7%)in females(P<0.05).The levels of GGT and ALT in males were significantly higher than those in females(P<0.05).Spearman correlation analysis illustrated that ALT and GGT levels were positively correlated to uric acid(r values were 0.175 and 0.434,P<0.01).Multivariate logistics regression showed that after adjusting age,body mass index,urban and rural area distribution,educational level,month income level,smoking and drinking,abnormal ALT of male(OR=1.64,95%CI:1.07~2.51)and female(OR=2.01,95%CI:1.34~3.00)or abnormal GGT of male(OR=2.87,95%CI:2.26~3.65)and female(OR=2.02,95%CI:1.45~2.81)were related to high risk of newly diagnosed HUA.Conclusion The abnormal levels of serum liver enzymes are related to the risk of newly diagnosed HUA in adult residents with different gender of Qingdao.
