联合化疗方案的序贯给药顺序及379份调查实例点评与分析

摘　　要：目的:总结常见联合化疗方案的适宜给药顺序,点评郴州市第一人民医院379份病例的联合化疗方案输液顺序情况,分析导致输液顺序不适宜的原因。方法:基于文献和药品说明书等总结联合化疗方案的适宜给药顺序,基于证据金字塔对文献证据级别分级。调查379份联合化疗患者的病例,确定药物给药时间,基于已建立的给药顺序规则点评调查实例并对输液顺序不适宜情况做原因分析。结果:总结了54种联合化疗方案的适宜给药顺序,部分文献证据级别偏中低级。379份病例共35种联合方案中,142份(37.47%)18种(51.43%)方案输液顺序不宜,原因与医生、药师和护士等有关。结论:联合化疗方案给药顺序的总结对临床合理输液予以了一定的参考。目前该院在联合化疗的输液顺序方面仍存在一定问题,应亟须干预以提高合理用药水平。OBJECTIVE To summarize appropriate administration sequence of co-chemotherapy regimens,and review379 surveyed cases in Chenzhou No.1 People′s Hospital and analyze causes of unsuitability.METHODS Collating of the appropriate administration sequence for co-chemotherapy regimen was performed based on literatures and package inserts,and evidence level of the literature was graded based on the evidence pyramid.379 cases of combined chemotherapy patients were investigated,the drug administration time was determined,the investigation examples were commented based on the established administration sequence rules,and the causes of inappropriate infusion sequence were analyzed RESULTS Administration sequence of 54 co-chemotherapy regimens were collated.Evidence level of some literatures located in middle and low grade.35 regimens in 379 patients were investigated,and 18(51.43%)regimens in 142(37.47%)patients had unsuitable infused sequences.The causes of unsuitability involved doctors,pharmacists and nurses.CONCLUSION Collating of the appropriate administration sequence for co-chemotherapy regimens provides a reference for rational infusion.Some problems in administration sequence have currently existed in our hospital,and urgent intervention should be needed to improve level of rational drug use.
