甘爱萍教授中医治疗慢性萎缩性胃炎患者的首诊思路探讨

摘　　要：目的:利用中医传承计算平台V3.0挖掘甘爱萍教授治疗慢性萎缩性胃炎患者的首诊思路,为慢性萎缩性胃炎患者的中医临床治疗提供一定的指导。方法:搜集2017年7月-2021年1月于湖北省中医院甘爱萍门诊就诊的慢性萎缩性胃炎患者(100例)的首诊资料,将所有患者的中药处方录入到中医传承计算平台,采用频次统计、聚类分析、关联规则等方法得出相关统计结果。结果:一共纳入处方100首,得到药物使用频次居前10位的中药为百合、茯苓、砂仁、蒲公英、石斛、白花蛇舌草、炒白术、合欢花、玫瑰花、郁金;功效以补虚类、清热类、理气类、利水渗湿类为主;四气以寒性最多,五味以甘味最多;分析出4个核心药对,3个核心组合。结论:甘教授认为慢性萎缩性胃炎患者证型多为"脾胃阴虚,气滞湿阻",常治以"滋阴清热,化湿行气",常用药物组合为百合、茯苓、砂仁、蒲公英、石斛、白花蛇舌草、炒白术、合欢花、玫瑰花、郁金。Objective:The Traditional Chinese Medicine Inheritance Computer System V3.0 was used to explore the first diagnosis ideas of Professor GAN Aiping in treating chronic atrophic gastritis,in order to provide certain guiding for treating chronic atrophic gastritis in TCM.Methods:The first visit data of patients(100 cases)from July 2017 to January 2021 was collected,and the prescriptions of all patients were input into the TCM Inheritance Computing Platform.The frequency statistics,cluster analysis,association rules and other methods were performed to obtain relevant statistical results.Results:A total of 100 prescriptions were included,and the top 10 traditional Chinese medicines were Baihe(Lily),Fuling(Poria),Sharen(Amomum villosum),Pugongying(Dandelion),Shihu(Dendrobium),Baihua Sheshecao(Oldenlandia diffusa),Chao Baizhu(Fried Atractylodes),Hehuanhua(Albizia Julibrissin),Meiguihua(Rose)and Yujin(Radix curcumae).The main effects were strengthening the body resistance,clearing heat,regulating Qi(气),and diminishing dampness.The property was mainly cold,and the flavor was mainly sweetness.There were 4 core medicine-pairs,and 3 core combinations.Conclusion:Professor GAN believes that the syndromes of chronic atrophic gastritis patients are mostly“spleen and stomach Yin(阴)deficiency,Qi stagnation and dampness”.The common treatment method is to nourish Yin and clear heat,dissipate dampness and promote Qi.The commonly used medicine combinations are Baihe,Fuling,Sharen,Pugongying,Shihu,Baihua Sheshecao,Chao Baizhu,Hehuanhua,Meiguihua and Yujin.
