黄氏壮医针灸配合“黄氏理筋”手法治疗退行性腰椎管狭窄症的临床研究

摘　　要：目的:探讨黄氏壮医针灸配合"黄氏理筋"手法治疗退行性腰椎管狭窄症(Degenerative Lumbar Spinal Stenosis,DLSS)的临床效果。方法:研究纳入玉林市中医医院收治的DLSS患者60例,研究时间在2020年2月-2021年1月。采用随机数字表法将60例患者分为对照组和观察组,各30例,其中对照组采用"黄氏理筋"手法治疗,而观察组采用黄氏壮医针灸配合"黄氏理筋"手法治疗,对比两组治疗效果。结果:对照组和观察组治疗前日本骨科协会评估法(Japanese Orthopaedic Association Scores,JOA)评分、腰椎Oswestry功能障碍指数(Oswestry Disability Index,ODI)评分以及视觉模拟评分法(Visual Analogue Scale,VAS)评分差异无统计学意义(P> 0.05);治疗2周后和4周后,两组上述三项评分较治疗前均有明显改善,组内对比差异有统计学意义(P <0.05),并且观察组治疗后上述三项评分均低于对照组,组间对比差异有统计学意义(P <0.05)。观察组总有效率明显高于对照组,差异有统计学意义(P <0.05)。观察组治疗前白细胞介素-6(Interleukin-6,IL-6)和肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)两项炎性反应递质,白细胞介素-4(Interleukin-4,IL-4)和白细胞介素-10(Interleukin-10,IL-10)两项抗炎细胞因子水平差异无统计学意义(P> 0.05);观察组治疗后IL-6和TNF-α水平低于对照组,IL-4和IL-10水平高于对照组(P <0.05)。结论:对DLSS患者采用黄氏壮医针灸配合"黄氏理筋"手法治疗可以显著改善其肢体功能障碍的情况,减轻患者的疼痛症状,从而显著提高患者的治疗效果,临床应用价值显著。Objective:To explore the clinical effects of HUANG’s acupuncture of Zhuang medicine(黄氏壮医针灸)plus HUANG’s tendon manipulation on degenerative lumbar spinal stenosis.Methods:60 cases from February 2020 to January 2021 were selected and divided into the control group and the observation group by the random number table method,30 cases in each group.The control group was treated by HUANG’s tendon manipulation,while the observation group was given HUANG’s acupuncture of Zhuang medicine plus HUANG’s tendon manipulation.Results:Before treatment,there was no statistically significant difference in JOA score,ODI score and VAS score between the two groups(P>0.05).In 2 weeks and 4 weeks after treatment,the above three scores in the two groups were ameliorated compared with those before treatment,with statistically significant differences within each group(P<0.05),and the above three scores in the observation group after treatment were lower than the control group,and P<0.05.The total efficiency in the observation group was significantly higher than the control group,P<0.05.After treatment,IL-6 and TNF-αlevels in the observation group were lower than the control group,and IL-4 and IL-10 levels in the observation group were higher than the control group(P<0.05).Conclusion:HUANG’s acupuncture of Zhuang medicine plus HUANG’s tendon manipulation on degenerative lumbar spinal stenosis can significantly improve limb dysfunction of patients,relieve pain symptoms,obviously improve therapeutic effect,and has significant clinical application value.
