消风止痒颗粒联合血液透析治疗尿毒症皮肤瘙痒症的疗效及对钙磷代谢的影响

摘　　要：目的:探讨消风止痒颗粒与血液透析联合治疗尿毒症皮肤瘙痒症的疗效,并观察其对钙磷代谢的影响。方法:选取山东省菏泽市单县中医医院2018年1月-2020年1月收治的80例尿毒症皮肤瘙痒症患者为研究对象,按照随机数字表法分为观察组及对照组(每组40例)。对照组实施常规西药联合血液透析治疗,观察组实施消风止痒颗粒结合血液透析治疗。观察两组患者的总有效率、不良反应、治疗前后的抑郁自评量表(Self-rating Depression Scale,SDS)评分、焦虑自评量表(Self-rating Anxiety Scale,SAS)评分、血钙、血磷、β2-微球蛋白、甲状旁腺激素、瘦素、瘙痒症状消失时间、肾脏疾病生活质量简表(Kidney Disease Quality of Life Short Form Questionnaire,KDQOL-SF)评分。结果:观察组总有效率显著高于对照组,P <0.05;治疗前,两组患者SAS、SDS评分差异不存在统计学意义,P> 0.05;治疗后,观察组SAS、SDS评分显著低于对照组,P <0.05;经治疗,两组患者血钙水平较治疗前显著提高,血磷水平较治疗前显著下降,但观察组的血钙、血磷水平显著优于对照组,P <0.05;治疗前,两组大分子毒素水平差异不存在统计学意义,P> 0.05;治疗后,观察组患者的瘦素、β2-微球蛋白、甲状旁腺激素显著低于对照组,P <0.05;较治疗前,两组生活质量评分均有所提升,但观察组36项简明健康状况调查量表(The Mos36-item Short From Health Survey,SF-36)评分、肾脏透析相关生存质量量表(Kidney Disease Targeted Areas Scale,KDTA)评分均显著高于对照组,P <0.05;治疗前,两组瘙痒评分差异无统计学意义,P> 0.05,治疗后,两组评分均有所降低,但观察组低于对照组,且瘙痒症状缓解时间短于对照组,P <0.05;观察组患者的总不良反应率显著低于对照组,P <0.05。结论:消风止痒颗粒联合血液透析治疗尿毒症皮肤瘙痒具有明显效果,可抑制大分子毒素,改善患者的心理状态及生活质量,安全Objective:To explore the curative effects of the Xiaofeng Zhiyang granules(消风止痒颗粒)plus hemodialysis on uremia skin itching,and to observe its influences on calcium and phosphorus metabolism.Methods:80 cases from January 2018 to January 2020 were selected as the research objects,and were divided into the observation group and the control group(40 cases in each group)by the random number table method.The control group was treated by conventional Western medicine plus hemodialysis,and the observation group was treated with the Xiaofeng Zhiyang granules plus hemodialysis.Results:The total efficiency in the observation group was significantly higher than the control group,P<0.05.After treatment,the SAS and SDS scores in the observation group were significantly lower than those in the control group,P<0.05.After treatment,the blood calcium level in the two groups were significantly higher than before treatment,and the blood phosphorus level in the two groups were obviously lower than before treatment.The blood calcium and blood phosphorus levels in the observation group were significantly better than the control group,P<0.05.After treatment,the leptin,β2-microglobulin,and parathyroid hormone levels in the observation group were significantly lower than those in the control group,P<0.05.Compared with before treatment,the scores of life quality in the two groups were increased,the scores of SF-36 and KDTA in the observation group were significantly higher the control group,P<0.05.After treatment,the itching score in the two groups were reduced,but in the observation group was lower than the control group,and the relief time of pruritus symptoms was shorter than the control group,P<0.05.The total adverse reaction in the observation group was significantly lower than the control group,P<0.05.Conclusion:The Xiaofeng Zhiyang granules plus hemodialysis shows obvious effects on uremia skin itching.It can inhibit macromolecular toxins,improve the psychological state and life quality of patients,and has high safety.
