化瘀丸对拇指外展功能重建患者术后炎症指标及肌腱粘连的影响

摘　　要：目的:探讨化瘀丸对拇指外展功能重建患者术后炎症指标及肌腱粘连的影响。方法:选取2018年6月-2019年6月于泉州市正骨医院接受手术治疗的91例拇指外展功能重建患者,依据随机数字表法将所有患者分为对照组(45例)与观察组(46例)。对照组术后采用常规治疗,观察组在对照组基础上加用化瘀丸治疗。比较患者中医症状积分、炎症指标、氧化应激指标、肌腱粘连发生情况,并随访1年评估患者的拇指功能。结果:治疗3个月后,两组患者的手指麻木、手部活动受限、指关节发凉、对掌无力评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P <0.05);治疗3个月后,两组血清白细胞介素-6(Interleukin-6,IL-6)、血清肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、白细胞介素-8(Interleukin-8,IL-8)水平均低于治疗前,丙二醛(Malondialdehyde,MDA)、羟基自由基(Hydroxyl Radical,-OH)、脂质过氧化物(Lipid Peroxide,LPO)亦低于治疗前,且观察组上述指标均低于对照组,差异有统计学意义(P <0.05);观察组肌腱粘连发生率略低于对照组,但差异无统计学意义(P> 0.05);随访1年,观察组拇指功能优良率高于对照组,差异有统计学意义(P <0.05)。结论:拇指外展功能重建患者术后采用化瘀丸治疗效果显著,可有效抑制机体炎性反应,减轻氧化应激反应,改善拇指功能,改善临床症状,且在一定程度上减少肌腱粘连的发生。Objective:To explore the effects of Huayu Wan(化瘀丸)on inflammation and tendon adhesion in patients after thumb abduction function reconstruction surgery.Methods:91 cases from June 2018 to June 2020 were selected and were divided into the control group(45 cases)and the observation group(46 cases)by the random number table method.The control group received conventional treatment after surgery,and the observation group was treated with Huayu Wan plus conventional treatment after surgery.Results:After 3 months of treatment,the scores of finger numbness,restricted hand movement,cold knuckles,and palm weakness in both groups were lower than before treatment,and in the observation group were lower than the control group,with statistically significant differences(P<0.05).After 3 months of treatment,the levels of serum IL-6,serum TNF-αand IL-8 in both groups were lower than before treatment;and MDA,-OH and LPO were lower than before treatment,and the above indexes in the observation group were lower than those in the control group,P<0.05.The incidence of tendon adhesion in the observation group was slightly lower than the control group,but the difference was not statistically significant(P>0.05).After 1 year of followed-up,the excellent and good of thumb function in the observation group was higher than the control group,P<0.05.Conclusion:Huayu Wan on thumb abduction function reconstruction surgery patients shows significant effects,it can effectively inhibit the body’s inflammatory reaction,reduce oxidative stress response,improve hand function,ameliorate clinical symptoms,and reduce the occurrence of tendon adhesion to a certain extent.
