尿激酶溶栓24小时内使用加味星蒌承气汤治疗急性脑梗死痰热腑实证的临床研究

摘　　要：目的:浅析急性脑梗死痰热腑实证患者尿激酶溶栓治疗24小时内使用加味星蒌承气汤的临床效果。方法:纳入北流市中医院2018年3月-2020年3月收治的急性脑梗死(痰热腑实证)38例,根据数字表法随机分为对照组与试验组,各19例,对照组接受尿激酶溶栓治疗,试验组基于此在24小时内开展加味星蒌承气汤治疗,就患者临床疗效进行组间比对。结果:与对照组相比,试验组患者远期神经缺损状况评分、中医证候积分均显著降低,差异有统计学意义(P <0.05);两组患者治疗前载脂蛋白A1、载脂蛋白B指标比较,差异无统计学意义(P> 0.05),但治疗后相比,两组患者载脂蛋白A1水平提升,载脂蛋白B水平降低,差异有统计学意义(P <0.05)。结论:针对急性脑梗死尿激酶溶栓患者,于24小时内给予加味星蒌承气汤,能有效提升治疗效果。Objective:To analyze the clinical effects of the Xinglou Chengqi decoction(星蒌承气汤)within 24 hours after urokinase thrombolysis on the phlegm-heat accumulation type acute cerebral infarction.Methods:38 cases from March 2018 to March 2020 were included.According to the number table method,all the included patients were randomly divided into the control group and the test group,19 cases for each.The control group received urokinase thrombolytic therapy,and the test group took the Xinglou Chengqi decoction within 24 hours after urokinase thrombolysis.Results:Compared with the control group,the long-term neurological deficit status score and TCM syndrome scores in the test group were significantly reduced,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in apolipoprotein A1 and apolipoprotein B between the two groups.After treatment,apolipoprotein A1 level was increased in the groups,apolipoprotein B level was decreased in the groups,and the difference was statistically significant(P<0.05).Conclusion:Treating the phlegm-heat accumulation type acute cerebral infarction with the Xinglou Chengqi decoction within 24 hours after urokinase thrombolysis can obviously improve the therapeutic effects.
