摘 要:目的探讨新生儿重症监护病房(NICU)病原菌分布情况及感染的高危因素。方法320例NICU新生儿根据病原菌检查结果分为感染组和未感染组,比较两组的临床资料,采用单因素和Logistic回归分析探讨感染的高危因素。结果320例NICU患儿的感染发生率为9.06%(29/320)。29例患儿中检出35株病原菌,其中革兰氏阴性菌占62.86%。感染组的胎龄<37周、住院时间>10 d、体质量<1500 g、静脉营养、侵入性操作、应用抗菌药物时间>7 d的比例均高于未感染组(P<0.05)。Logistic回归分析显示,住院时间>10 d、静脉营养、侵入性操作、应用抗菌药物时间>7 d是NICU院内感染发生的高危因素(P<0.05)。结论NICU病原菌以革兰氏阴性菌为主,临床应加强识别潜在危险因素,采取针对性措施,降低新生儿感染的风险。Objective To explore the distribution of pathogenic bacteria and high risk factors of infection in neonatal intensive care unit(NICU).Methods 320 neonates in NICU were divided into the infection group and the non-infection group according to the results of pathogenic bacteria examination.Univariate analysis and Logistic regression analysis were used to explore the high risk factors of infection.Results The infection rate of 320 NICU neonates was 9.06%(29/320).35 strains of pathogenic bacteria were detected in 29 neonates,among which Gram-negative bacteria accounted for 62.86%.The proportions of gestational age<37 weeks,hospitalization time>10 days,body weight<1500 g,intravenous nutrition,invasive procedures,and antibiotics application time>7 days in the infection group were higher than those in the non-infection group(P<0.05).Logistic regression analysis showed that hospitalization time>10 days,intravenous nutrition,invasive procedures,and antibiotics application time>7 days were high risk factors for the occurrence of infection in NICU(P<0.05).Conclusions The pathogenic bacteria in NICU are mainly Gram-negative bacteria.The clinical identification of potential risk factors should be strengthened,and targeted measures should be taken to reduce the risk of neonatal infection.
