广东省脑死亡质控数据回顾分析及策略探讨

摘　　要：目的探讨大体量脑死亡病例质控策略,促进脑死亡判定工作规范化。方法从国家脑死亡判定质控信息平台提取相关数据,回顾性分析2013年—2020年广东省脑损害评估质量控制中心脑死亡判定工作量及质控相关指标。结果广东省中心脑死亡判定质控体量呈增长趋势。2020年广东省中心质控病例数为52.05%。2017年—2020年广东省质控中心临床评估符合率从0提升至97.18%,自主呼吸激发试验实施率从23.08%提升至98.50%,脑电图评估符合率从26.67%提升至99.26%,短潜伏期体感诱发电位评估符合率从30.77%提升至99.24%,经颅多普勒超声评估符合率为57.14%~61.63%,两项确认试验覆盖率为93.33%~99.58%。结论广东省脑死亡病例质控体量逐年增长。质控工作需要质控人员专职化,质控系统信息化,质控管理精细化,以进一步提高大体量脑死亡病例质控效率和质量。Objective To explore the quality control strategy of large numbers of brain death cases and promote the standardization of brain death determination.Methods Relevant data were extracted from the national brain death determination quality control network system,and the workload of brain death determination and quality control indexes of Guangdong Center for Brain Injury Assessment and Quality Control from 2013to 2020were retrospectively analyzed.Results The quality control volume of brain death determination of Guangdong provincial center showed an overall increasing trend.In 2020,the number of quality control cases in Guangdong provincial center was 52.05%.From 2017to 2020,the compliance rate of clinical evaluation in Guangdong provincial center increased from 0%to 97.18%,the implementation rate of opnea test increased from 23.08%to 98.50%,and the compliance rate of electroencephalography assessment increased from 26.67%to 99.26%.The coincidence rate of short latency somatosensory evoked potentials assessment was increased from 30.77%to 99.24%,and the coincidence rate of transcranial doppler ultrasound assessment was 57.14%~61.63%.The coverage rate of the two confirmed tests was93.33%~99.58%.Conclusion The volume of quality control of brain death cases has increased in Guangdong Province.It is suggested to establish full-time quality control team,strengthen information construction of quality control system and improve quality control management system,so as to further improve the efficiency and quality of quality control of general brain death cases.
