实脾消水膏外敷联合艾灸治疗胃癌腹水患者的效果分析

摘　　要：目的:分析实脾消水膏外敷联合艾灸治疗胃癌腹水患者的效果。方法:选取2018年6月—2020年6月我院收治的76例胃癌腹水患者作为研究对象,依据随机数字表法分为传统组和联合组,每组38例。传统组应用腹腔灌注顺铂注射液治疗,联合组在传统组基础上加用实脾消水膏外敷联合艾灸治疗,比较两组临床效果、不良反应发生情况、中医症候积分、生活质量,腹水中白介素-2(IL-2)浓度及NK细胞、CD4^(+)/CD8^(+)水平;并随访6个月,分析两组患者生存状况。结果:联合组总有效率为97.37%,高于传统组的78.95%,差异有统计学意义(P<0.05)^(+)联合组气短、腹满、疼痛、食欲减退、食后腹胀评分低于传统组,差异有统计学意义(P<0.05);联合组腹水中IL-2、NK细胞、CD4^(+)/CD8^(+)水平高于传统组,差异有统计学意义(P<0.05);联合组躯体功能、认知功能、角色功能、社会功能、情绪功能、总体健康评分高于传统组,差异有统计学意义(P<0.05);随访6个月,联合组生存率为97.44%,高于传统组的76.32%,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:应用实脾消水膏外敷联合艾灸治疗胃癌腹水患者效果较佳,可降低患者中医症候积分,降低腹水中IL-2、NK细胞、CD4^(+)/CD8^(+)水平,提高生活质量和生存状况,具有应用有效性及安全性。Objective:To analyze the effect of external application of Shipi Xiaoshui ointment combined with Moxibustion in the treatment of gastric cancer ascites.Methods:A total of 76 patients with gastric cancer ascites admitted to our hospital from June 2018 to June 2020 were selected as the research subjects.According to the random number table method,they were divided into the traditional group and the combined group,with 38 cases in each group.The traditional group was treated with intraperitoneal injection of cisplatin,and the combined group was treated with external application of Shipi Xiaoshui ointment combined with Moxibustion on the basis of the traditional group.The clinical effect,adverse reactions,TCM symptom score,quality of life,and interleukin-2(IL-2)concentration,NK cells and CD4^(+)/CD8^(+)levels in ascites were compared between the two groups.The patients were followed up for 6 months to analyze the survival status of the two groups.Results:The total effective rate of the combined group was 97.37%,which was higher than 78.95%of the traditional group,the difference was statistically significant(P<0.05).The scores of shortness of breath,abdominal fullness,pain,loss of appetite and abdominal distension after eating in the combined group were lower than those in the traditional group,the difference was statistically significant(P<0.05).The levels of IL-2,NK cells and CD4^(+)/CD8^(+)in ascites in the combined group were higher than those in the traditional group,the difference was statistically significant(P<0.05).The physical function,cognitive function,role function,social function,emotional function and overall health score of the combined group were higher than those of the traditional group,the difference was statistically significant(P<0.05).After 6 months of follow-up,the survival rate of the combined group was 97.44%,which was higher than 76.32%in the traditional group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse re
