经皮螺钉固定治疗第五跖骨Jones骨折的疗效观察

摘　　要：目的经皮采用无头加压空心螺钉髓内固定治疗分析第五跖骨Jones骨折的临床体会。方法随机选取郑州市骨科医院2012年1月至2018年1月收治的55例第五跖骨Jones骨折患者,均给予经皮无头加压空心螺钉进行髓内固定治疗,通过分析观察患者的伤口愈合情况、骨折愈合情况、愈合时间,在治疗前后采用美国足踝外科协会(AOFAS)进行足部功能评分以评估足部功能恢复情况。结果在随访期末,所有患者足部功能评分明显优于治疗前(P<0.05)。55例患者伤口均达到Ⅰ期愈合,骨折在2.5~3.5个月愈合,随访期间,所有患者未出现内置物断裂、松动和足部畸形等并发症。结论经皮无头加压空心螺钉髓内固定是治疗第五跖骨Jones骨折的有效方法。Objective To analyze the clinical experience of Jones fractures of the fifth metatarsal bone with percutaneous use of headless compression hollow screws.Methods A random selection of 55 cases of Jones fractures of the fifth metatarsal bone admitted to our hospital from January 2012 to January 2018 were given percutaneous headless compression hollow screws for intramedullary fixation,and the wound healing of the patients was observed through analysis,Fracture healing status、healing time,before and after treatment,the American Society of Foot and Ankle Surgery(AOFAS)was used to score the foot function to evaluate the recovery of foot function.Results At the end of the follow-up period,the foot function scores of all patients were significantly better than before treatment(P<0.05).The wounds of the 55 patients all achieved first-stage healing,and the fractures healed within 2.5-3.5 months.During the follow-up period,there were no complications such as broken fixation,loosening,and foot deformity in all patients.Conclusion As an important part of the lateral longitudinal arch of the foot,the fifth metatarsal has less blood supply.Conservative treatment may cause complications such as fracture nonunion,delayed union,and chronic pain.Percutaneous use of headless compression hollow screw intramedullary Fixation treatment of the fifth metatarsal Jonse fracture can achieve firm fixation,effectively avoid foot pain,and avoid foot function loss.It can be used as an effective method for treating Jones fractures at the base of the fifth metatarsal.
