Meta-analysis of Blue light combined with yinzhihuang and bifidobacteria preparations in the treatment of neonatal hyperbilirubinemia

摘  要:目的:系统评价益生菌联合茵栀黄及蓝光治疗新生儿高胆红素血症的疗效及安全性。方法:检索建库至今中国学术期刊全文数据库(CNKI)、万方数据库、维普数据库(VIP)、Cochrane Library、Embase、PubMed等数据库发表的有关益生菌联合茵栀黄及蓝光治疗的随机对照试验(RCT)文献,应用RevMan 5.4进行Meta分析。结果:共纳入8篇RCTs,其中观察组(益生菌联合茵栀黄及蓝光治疗)和对照组(蓝光治疗)分别为489例和478例。Meta分析结果显示:观察组的总治愈率高于对照组[RR=5.83,95%CI(3.10,10.97),P<0.00001]、黄疸消退时间观察组短于对照组[MD=-5.15,95%CI(-6.05,-4.24),P<0.00001]、治疗后胆红素浓度观察组明显低于对照组[MD=-38.25,95%CI(-52.91,-23.58),P<0.00001]、不良反应率观察组与对照组差异无统计学意义[RR=0.75,95%CI(0.26,2.16),P=0.59]。结论:益生菌联合茵栀黄及蓝光可提高治疗新生儿高胆红素血症的疗效,缩短黄疸持续时间,且不增加不良反应的发生。Objective:To systematically evaluate the efficacy and safety of Blue light combined with yinzhihuang and bifidobacteria preparations in the treatment of neonatal hyperbilirubinemia.Methods:A randomized controlled trial(RCT)of Blue light combined with Yinzhihuang and Bifidobacteria preparations published by CNKI,Wanfang,VIP,Cochrane Library、Embase、PubMed was retrieved and Meta analysis was performed using RevMan 5.4.Results:A total of 8 RCTs were included,including 489 cases in the observation group(Blue light combined with yinzhihuang and bifidobacteria preparations)and 478 cases in the control group(Blue light administration).Meta analysis results showed that the total cure rate in the bservation group was higher than that in the control group[RR=5.83,95%CI(3.10,10.97),P<0.00001),the jaudice faded time in the observation group was shorter than that in the control group[MD=-5.15,95%CI(-6.05,-4.24),P<0.00001),the bilirubin concentration in the observation group after treatment was significantly lower than that in the control group[MD=-38.25,95%CI(-52.91,-23.58),P<0.00001),and the adverse reaction rate of observation group and control group had no statistical differences[RR=0.75,95%CI(0.26,2.16),P=0.59].Conclusion:Blue light combined with yinzhihuang and Bifidobacteria can improve the curative effect,shorten the duration of jaundice,and do not increase the incidence of adverse reactions.
