摘  要:目的通过靶向代谢组学分析儿童特应性皮炎(atopic dermatitis,AD)急性发作期的血浆氨基酸代谢变化,从代谢通路方面探讨特应性皮炎发病机制;探讨特应性皮炎亚型外源型和内源型急性发作期血浆氨基酸代谢差异。方法将收集的符合儿童特应性皮炎诊断标准的患者40例(其中22例为外源型AD组,18例为内源型AD组)和非特应性皮炎患者为对照组17例作为研究对象,采用代谢组学检测血浆样本中的代谢物质,利用主成分分析法(PCA)及偏最小二乘-判别分析法(PLA-DA)对产生的矩阵数据进行分析,筛选出血浆样本差异性代谢物质,通过MetaboAnalyst 5.0平台分析可能存在的异常代谢通路。结果各组间区分良好,筛选出差异代谢物及代谢通路。AD组与对照组间存在7种代谢差异物,外源型与内源型AD组存在6种代谢差异物;比较AD组与对照组发现存在的异常代谢通路有精氨酸和脯氨酸代谢,乙醛酸和二羧酸代谢,丙氨酸、天冬氨酸和谷氨酸代谢;内源型和外源型AD组发现存在的异常代谢通路有氨基酰生物合成,组氨酸代谢,丙氨酸、天冬氨酸和谷氨酸代谢,乙醛酸和二羧酸代谢,甘氨酸、丝氨酸和苏氨酸代谢。结论儿童特应性皮炎急性发作期与对照组的血浆氨基酸代谢物具有明显差异,可能与特应性皮炎的发病机制相关;急性发作期内源型和外源型特应性皮炎血浆部分氨基酸代谢及代谢通路具有代谢差异。Objective To investigate the changes of plasma amino acid metabolism in children with atopic dermatitis(AD)during acute attack by targeted metabonomics,and to explore the pathogenesis of AD from the perspective of metabolic pathways,and to analyze the difference of plasma amino acid metabolism between extrinsic and intrinsic AD subtype during acute attack.Methods A total of 40 patients(including 22 patients with extrinsic AD and 18 patients with intrinsic AD)who met diagnostic criteria for AD and 17 patients without AD as control group were collected as subjects.Metabolomics was used to detect the metabolites in plasma samples.Principal Component Analysis(PCA)and Partial Least Squares-discriminant Analysis(PLA-DA)were used to analyze the metabolites.The matrix data generated were analyzed to screen out differential metabolites in plasma samples,and the possible abnormal metabolic pathways were analyzed through MetaboAnalyst 5.0 platform.Results The groups were well distinguished,and the difference metabolites and metabolic pathways were sereened out.There were 7 kinds of differential metabolites between the AD group and control group,and 6 kinds of differential metabolites between the extrinsic AD group and the intrinsic AD group.The abnormal metabolic pathways in AD group and control group were found to be alanine,aspartate and glutamate metabolism,glyoxylate and dicarboxylate metabolism,and arginine and proline metabolism.The abnormal metabolic pathways found in the extrinsic and intrinsic AD groups included aminoacyl-tRNA biosynthesis,histidine metabolism,alanine,aspartate and glutamate metabolism,glyoxylate and dicarboxylate metabolism,and glycine,serine and threonine metabolism.Conclusion There are significant differences in plasma amino acid metabolites between the control group and children during the acute attack of AD,which may be related to the pathogenesis of AD.During the acute attack period,some amino acid metabolism and metabolic pathways in plasma of extrinsic and intrinsic AD are different.
