系统性淀粉样变导致扁平苔藓样甲改变1例并文献复习

摘　　要：患者男,58岁,四肢甲板变薄,纵纹4年,活动后呼吸困难2年,加重10 d。皮肤科情况:舌体稍肥大,舌边缘见齿痕,双手指甲及双足趾甲均可见甲板萎缩变薄、纵纹。骨髓穿刺送检:浆细胞明显增加,浆细胞占20.5%,幼浆13%。腹部脂肪、膀胱、腹膜后组织活检刚果红染色均为阳性。诊断:①多发性骨髓瘤;②原发性系统性淀粉样变性。A 58-year-old man presented with a 4-year history of thinning and longitudinal ridging of twenty nails,and a 2-year history of difficulty in breathing after activity.These symptoms could become apparent after activity and worsen in the last 10 days.Further dermatological examination showed a slight hypertrophy of the tongue,teeth marks on the edge of the tongue,and atrophy and thinning of the nail plate and vertical lines on the nails of both hands and toenails.A bone marrow biopsy showed a significant increase in plasma cell(20.5%),as compared with 13%proplasmacyte.Abdominal fat biopsy,bladder and retroperitoneal tissue biopsy were positive for Congo red staining.Diagnosis of multiple myeloma and primary systemic amyloidosis was made.
