平乐郭氏荣肌揉筋法治疗膝关节置换术后下肢肿胀临床研究

摘　　要：目的:观察平乐郭氏荣肌揉筋法治疗膝关节置换术(total knee arthroplasty,TKA)术后下肢肿胀的临床疗效。方法:将60例TKA术后下肢肿胀患者按照随机数字表法分为观察组和对照组,每组各30例。对照组在TKA术后给予传统康复训练,观察组在对照组治疗的基础上加用平乐郭氏荣肌揉筋法。比较两组患者术前1 d、术后第7天及术后第14天患肢肿胀程度、疼痛程度及血浆D-二聚体水平变化情况。结果:两组患者术后第7天肿胀程度大于术前1 d,术后第14天肿胀程度小于术后第7天,且观察组小于对照组,差异有统计学意义(P<0.05)。两组患者术后第7天及第14天疼痛视觉模拟评分低于术前1 d,且术后同期观察组低于对照组,差异有统计学意义(P<0.05)。两组患者术后第7天D-二聚体高于术前1 d,术后第14天D-二聚体低于术后第7天,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:平乐郭氏荣肌揉筋法能有效减轻TKA术后下肢肿胀及疼痛程度,降低血浆D-二聚体水平。Objective:To observe the clinical curative effect of Pingle Master Guo′s method of nourishing muscles and kneading tendons on swelling of lower limbs after total knee arthroplasty(TKA).Methods:A total of 60 patients with lower limb swelling after TKA were randomly divided into the observation group and the control group,with 30 cases in each group.The control group was given traditional rehabilitation training after TKA,and the observation group was added with Pingle Master Guo′s method of nourishing muscles and kneading tendons on the basis of the treatment of the control group.The changes of swelling degree,pain degree and plasma D-dimer level of affected limbs 1 day before operation,7 days after operation and 14 days after operation were compared between the two groups.Results:The degree of swelling on the 7 th day after operation was greater than that on the 1 st day before operation,the degree of swelling on the 14 th day after operation was less than that on the 7 th day after operation,and the degree of swelling on the 14 th day after operation in the observation group was less than that in the control group,and all the differences were statistically significant(P<0.05).The pain visual analogue scale(VAS) score of the two groups on the 7 th and 14 th day after operation was lower than that of the control group respectively of the same period,and the differences were statistically significant(P<0.05).D-dimer in the observation group was lower than that in the control group 14 days after operation,and the difference was statistically significant(P<0.05).Conclusion:Pingle Master Guo′s method of nourishing muscles and kneading tendons can effectively relieve the swelling and pain after TKA and reduce the level of plasma D-dimer.
