大疱性类天疱疮患者白细胞介素-31、白细胞介素-16的表达水平及其与瘙痒的关系

摘　　要：目的观察大疱性类天疱疮(BP)患者血清及疱液中白细胞介素-31(IL-31)、白细胞介素-16(IL-16)水平,探讨这两者在BP瘙痒发生中的作用与意义。方法选取2019年10月至2021年5月于郑州大学第一附属医院皮肤科首次确诊的22例BP患者,纳入BP组。选取37例同期于该院皮肤科就诊的其他患者作为对照组。采用视觉模拟量表(VAS)评估瘙痒程度。留取BP患者外周血、疱液及对照组的外周血。采用酶联免疫吸附法检测血清、疱液中IL-31、IL-16的水平。比较BP组与对照组血清IL-31和IL-16的水平。分析血清和疱液IL-31、IL-16水平与瘙痒程度的相关性。结果BP组VAS评分较对照组高(P<0.05)。两组血清IL-16水平比较,差异无统计学意义(P>0.05);BP组血清IL-31水平高于对照组(P<0.05)。BP患者血清IL-31、IL-16水平与瘙痒程度呈正相关(γ_(s)=0.706、0.691,P<0.05)。BP患者疱液IL-31、IL-16水平与瘙痒程度呈正相关(γ_(s)=0.900、0.861,P<0.05)。BP患者血清IL-31水平与血清IL-16水平呈正相关(γ_(s)=0.967,P<0.05)。BP患者疱液IL-31水平与疱液IL-16水平呈正相关(γ_(s)=0.965,P<0.05)。结论BP患者血清和疱液中IL-31、IL-16的表达与瘙痒程度有关,并可能参与了BP瘙痒的发生和持续过程。Objective To observe the levels of interleukin-31(IL-31)and interleukin-16(IL-16)in serum and blister fluid of patients with bullous pemphigoid(BP),and to explore the role and sigmificance of IL-31 and.IL-16 in pruritus of BP.Methods From October 2019 to May 2021,22 patients with BP who were first diagnosed in the Department of Dermatology of the First Affiliated Hospital of Zhengzhou University were selected and included in BP group.And 37 other patients in the Department of Dermatology of the hospital in the same period were selected as control group.Visual analogue scale(VAS)was used to evaluate the degree of pruritus.Peripheral blood and blister fluid of BP patients and peripheral blood of control group were collected.The levels of IL-31 and IL-16 in serum and blister fluid of all specimens were detected by enzyme linked immunosorbent assay,and the levels ofIL-31 and IL-16 in serum of BP group and control group were compared.The correlations between the levels of IL-31 and IL-16 in serum and blister fluid with pruritus degree were analyzed.Results The VAS score of BP group was higher than that of control group(P<0.05).There was no statistical difference in serum IL-16 level between the two groups(P>0.05).The serum IL-31 level in BP group was higher than that in control group(P<0.05).The levels of IL-31 and IL-16 in serum of BP group were positively correlated with pruritus degree(γ_(s)=0.706,0.691,P<0.05).The levels of IL-31 and IL-16 in blister fluid of BP group were positively correlated with pruritus degree(γ_(s)=0.900,0.861,P<0.05).The level of serum IL-31 in BP patients was positively correlated with the level of serum IL-16(γ_(s)=0.967,P<0.05).The level of IL-31 in blister fluid of BP patients was positively correlated with the level of IL-16 in blister fluid(γ_(s)=0.965,P<0.05).Conclusion The expressions of IL-31 and IL-16 in serum and blister fluid of BP patients are related to the degree of pruritus and may be involved in the occurrence and persistence of prutitus of BP.
