地特胰岛素联合门冬胰岛素治疗妊娠糖尿病的临床效果

摘　　要：目的分析地特胰岛素联合门冬胰岛素治疗妊娠糖尿病的临床效果。方法选取2019年1—12月河南中医药大学第一附属医院收治的63例妊娠糖尿病患者。按治疗方法将患者分为对照组(30例)和观察组(33例)。对照组患者接受地特胰岛素治疗,观察组患者接受地特胰岛素联合门冬胰岛素治疗。记录患者血糖达标时间、胰岛素使用剂量、空腹血糖、餐后2h血糖以及低血糖发生事件。比较两组治疗前后网膜素1、同型半胱氨酸(Hcy)和稳态模型的胰岛素抵抗(HOMA-IR)指数。结果观察组空腹血糖达标时间、餐后2h血糖达标时间均短于对照组,胰岛素使用剂量少于对照组(P<0.05)。治疗后,两组空腹血糖和餐后2h血糖下降,观察组空腹血糖和餐后2h血糖低于对照组(P<0.05)。观察组低血糖发生率与对照组比较,差异无统计学意义(P>0.05)。治疗后,两组网膜素1水平高于治疗前,Hcy水平、HOMA-IR指数低于治疗前(P<0.05);治疗后,观察组网膜素1水平高于对照组,Hcy水平、HOMA-IR指数低于对照组(P<0.05)。结论地特胰岛素联合门冬胰岛素对妊娠糖尿病有明显作用,血糖控制快,低血糖事件少。Objective To analyze the clinical effect of insulin detemir combined with insulin aspart on gestational diabetes mellitus.Methods A total of 63 cases of gestational diabetes mellitus collected from the First Affliated Hospital of Henan University of Traditional Chinese Medicine from January to December 2019 were selected.The patients were divided into control group(30 cases)and observation group(33 cases)according to the treatment methods.The patients in control group were treated with insulin detemir,while the patients in observation group were treated with insulin detemir combined with insulin aspart.The time of reaching the standard of blood glucose,insulin dosage,fasting blood glucose,2-hour postprandial blood glucose and hypoglycemia were recorded.Omentin-1,homocysteine(Hey)and the index of homeostasis model assessment of insulin resistance(HOMA-IR)were compared between the two groups before and after treatment.Results The time of reaching the target of fasting blood glucose and 2-hour postprandial bood glucose in observation group were shorter than those in.control group,and the dosage of insulin was less than that in control group(P<0.05).After treatment,the fasting blood glucose and 2-hour postprandial blood glucose in the two groups decreased,while the fasting blood glucose and 2-hour postprandial blood glucose in observation group were lower than those in control group(P<0.05).The incidence of hypoglycemia in observation group was not significantly different from that in control group(P>0.05)。After treatment,the levels of Omentin-1 in the two groups were higher than those before treatment,while the levels of Hey and HOMA-IR were lower than those before treatment(P<0.05).Afer treatment,the level of Omentin-1 in observation group was higher than that in control group,while the levels of Hcy and HOMA-IR index were lower than those in control group(P<0.05).Conclusion Insulin detemir combined with insulin aspartate is effective in the treatment of gestational diabetes mellitus,with rapid control of blood s
