摘　　要：目的分析卵巢囊肿患者腹腔镜术后自然流产的影响因素。方法选取2013年8月至2020年8月周口市人民医院收治的115例接受腹腔镜卵巢囊肿剥除术治疗的患者。根据患者术后妊娠结局分为流产组(23例)和未流产组(92例)。采用多因素logistic回归分析卵巢囊肿患者腹腔镜术后自然流产的影响因素。分析术前糖类抗原125(CA125)水平、术后生育指数评分对卵巢囊肿患者腹腔镜术后自然流产的预测价值。结果年龄≥35岁、子宫内膜异位囊肿、术后妊娠时间、囊肿多发、双侧囊肿、术前CA125水平>62.75U·mL^(-1)、单孔手术、术中电凝止血、术后生育指数评分<6.04分是卵巢囊肿患者腹腔镜术后自然流产的影响因素(P<0.05)。术前CA125水平、术后生育指数评分联合预测卵巢囊肿患者腹腔镜术后自然流产的曲线下面积(AUC)最大,敏感性、特异性较高。结论卵巢囊肿患者腹腔镜术后自然流产受年龄、囊肿性质、术后妊娠时间、囊肿数目、囊肿侧别、术前CA125水平、手术方式、术中止血方式及术后生育指数评分等因素的影响。术前CA125水平与术后生育指数评分联合对评估自然流产发生率具有良好的预测价值。Objective To analyze the influencing factors of spontaneous abortion in patients with ovarian cyst after laparoscopic surgery.Methods From August 2013 to August 2020,115 patients in Zhoukou People’s Hospital who received laparoscopic ovarian cyst removal were selected.The patients were divided into abortion group(23 cases)and non-abortion group(92 cases)according to the pregnancy outcome after operation.Multivariate logistic regression was used to analyze the influencing factors of spontaneous abortion in patients with ovarian cyst after laparoscopic surgery.The predictive value of preoperative carbohydrate antigen 125(CA125)level and postoperative fertility index score on spontaneous abortion in patients with ovarian cyst after laparoscopic surgery were analyzed.Results Age≥35 years old,endometriotie cysts,postoperative pregnancy time,multiple cysts,bilateral cysts,preoperative CA125 level>62.75 U·mL^(-1),single port surgery,intraoperative electrocoagulation hemostasis and postoperative fertility index score<6.04 points were influencing factors of spontaneous abortion in patients with ovarian cysts after gynecological laparoscopy(P<0.05).The area under curve(AUC)of the preoperative CA125 level combined with postoperative fertility index score to predict spontaneous abortion in patients with ovarian cyst after laparoscopic surgery was the largest,with high sensitivity and specificity.Conclusion The spont aneous abortion of patients with ovarian cyst after laparoscopic surgery is influenced by factors such as age,cyst nature,postoperative pregnancy time,cyst number,cyst side,preoperative CA125 level,operation mode,intraoperative hemostasis mode and postoperative fertility index score.The combination of preoperative CA125 level and postoperative fertility index score has a good predictive value for evaluating the incidence of spontaneous abortion.
